Qualifications : Licensing : PLPC, MSW, RSW or CSW .Excellent communication and interpersonal skills. Previous supervision experience preferably in a social work, behavioral health setting.
Writing skills are necessary to perform the job.
Must have experience in the mental health field.
Position Summary : Implements the client care as directed by the treatment team, responsible for conducting individual and group therapies, participating in treatment planning, assisting clients and family members with psychosocial and financial issues and discharge planning.

CHARACTERISTIC DUTIES :
- Provide mental health services (i.e., CPST, PSR, Psychotherapy, and / or Crisis Intervention) to adults, adolescents, children, and families at the behest of the clients individualized treatment plan(s).
- Function as the client's advocate in all treatment decisions affecting the client.
- Support clients in preparation of follow-up visits for consultation or re-evaluation.
- Complies with HIPAA regulations.
- Shall be on duty and on call as needed.
- Attends and participates in weekly staff meetings, trainings, supervision, periodic meetings, treatment planning and discharge planning in-person, conference calls, or virtual meetings to ensure integrated services, reduce gaps in care, and address any problems that arise.
- Abides by outlined timeframes to submit Intake Paperwork, Monthly Progress Reports, Progress Notes, Supervision Logs, 90-Day Summaries, etc.
- Use tests, measurements, and measurement scale for general and specific assessments as well as objective client criteria in the provision of services and evaluation procedures.
- Assists in the development and is involved in the implementation of programs to assist clients and their families with the social and psychological factors of illness.
- Engages in individual or group counseling sessions pertaining to care and adjustment.
- Arranges for discharge and aftercare planning.
- Coordinates the social services discharge and home care planning process to facilitate the continuity of care and the provision of effective client and family support services.
- Coordinates the provision of information and guidance to clients, their families and other hospital departments regarding psychosocial aspects of client's post-discharge treatment.
- Coordinates the referral of clients and families to appropriate community, social service, home health services, extended care and rehabilitation facilities and organizations.
- Assists clients, families, and significant others with financial issues with appropriate financial aid resources when indicated.
- Coordinates the provision of social services to clients, families, and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from health care services.
- Coordinates, plans, and presents in-service education for other personnel and outside groups as requested.
- Complete all documentation or other documents related to recovery, supervision, phone contact, and report findings to the Liaison, Clinical Director, and / or Clinical Supervisor.
- Address any and all Human Resources concerns (i.e., issues with EHR system, leave requests, payroll questions, etc.) with the Administrator or presiding office staff.
- Initial and annual assessment, including the LOCUS / CALOCUS.
- Develop a treatment plan in collaboration with the member and family if applicable (or other collateral contacts) on the specific strengths and needs, resources, natural support(s), and individual goals and objectives for the member.
- Monitor ongoing needs of clients including triggering an update of the treatment plan if needs change significantly.
- Provides direct care to clients and may serve as counselor to specified caseload under clinical supervision.
- Notifies immediate supervisor and / or Clinical Director when caseload has reached maximum capacity to effectively perform quality care to clients.
- Performs other duties assigned.
LMHP providing CPST will receive regular scheduled clinical supervision from the Clinical Director or Clinical Supervisor and will adhere to the following :
- Assist the client with effectively responding to or avoiding identified precursors or triggers that would risk the client remaining in a natural community location.
- Assist in the development of daily living skills specific to managing a home.
- Assist the client and family members to identify strategies or treatment options associated with the client's mental illness.
LMHP providing Crisis Intervention Services will adhere to the following :
- Perform the preliminary screening.
- Immediately consult the Clinical Supervisor, Clinical Director, or LMHP for guidance.
- Provide short term crisis intervention, including crisis resolution and debriefing with the client.
- Contact family members when necessary.
- Submit Crisis Intervention (CI) Note in EHR system within 24hrs of service being rendered.
- Follow up with the client and as necessary with family members and / or caretaker.
LMHP providing PSR will receive regular scheduled clinical supervision from the Clinical Director or Clinical Supervisor and will adhere to the following :
- Perform an intervention via face-to-face or telehealth with the client present with the indented purpose of achieving the identified goals or objectives as set forth in the clients individualized treatment plan.
- Services may be provided individually or in a group setting.
- The size of the group therapy does not exceed 15 adults or 8 adolescents or children.
QUALIFICATIONS :
- Self-motivated, capable of being a resource to staff and working under pressure in certain cases.
- Capable of Multi-tasking, Listening, and has excellent communication skills.
