Your Role :
- Compassionate Care : Provide expert nursing care and emotional support to patients and their families.
- Holistic Assessments : Conduct comprehensive assessments to understand and address patients' physical, emotional, and spiritual needs.
- Personalized Planning : Develop tailored care plans in collaboration with patients and their loved ones, ensuring comfort and dignity throughout their hospice journey.
- Coordination Services : Coordinate services and resources to optimize patient care and support.
- Empowerment Education : Educate patients and families about hospice services, disease progression, and end-of-life care options.
- Clinical Excellence : Administer medications, manage symptoms, and adhere to clinical protocols with precision and empathy.
- Team Collaboration : Work closely with physicians, nurse practitioners, social workers, and other healthcare professionals to enhance patient outcomes and quality of life.
Qualifications :
- Registered Nurse (RN) license in good standing.
- Excellent communication and problem-solving skills.
- Ability to work independently and manage time effectively.
- A compassionate and patient-focused approach to care.
