Job Description
Starting in Alexandria will transfer to Fairfax - The Inova Cares for Community Bridging (ICCB) Program Coordinator ensures seamless transitions of care for patients post-hospitalization or Emergency Department (ED) visits.
This role involves managing inbound patient referrals, proactive patient engagement, and real-time cross-departmental collaboration for optimal patient outcomes.
The Coordinator also promotes the ICCB program through education efforts and establishing / maintaining community partnerships.
Strong verbal and written communication skills, interpersonal skills, education facilitation experience, and team-building skills are essential for effective collaboration with patients and the ICCB care team.
Must be Bilingual
On-site Monday through Friday, 8 : 00 AM - 5 : 00 PM, with flexible start times between 8 : 00 AM and 8 : 30 AM
Job Responsibilities
- Manages Inbound Referrals : Serves as the primary contact for post-hospital patient follow-up including, triaging referrals for appropriateness, scheduling appointments, connecting ineligible patients to other resources as needed, communicate referral processes to patients, families, and healthcare providers, and coordinate follow-up appointments with ICCB clinics and community partners.
- Cross-collaboration : Communicates and collaborates with other Inova departments and external clinics / organizations to ensure smooth transitions of care.
Acts as a Liaison between staff, interdisciplinary team members, patients, families, hospitals and clinics.
Program education / outreach : Fosters community partnerships and maintains a strong understand of ICCB eligibility criteria, providing education to internal and external partners, as needed.
Promotes the ICCB program to increase participation, and evaluates program goals in collaboration with departmental staff.
- Program compliance : Tracks referral data, maintains rolling reports, adheres to HIPAA and patient confidentiality requirements, and completes necessary documentation in compliance with healthcare regulations.
- Financial Counseling : Keeps up with changing payer markets, payments, and requirements. Maintains a strong understanding of the Inova Financial Assistance policy including criteria and provides basic financial counseling to patients, as needed.
- Educational Initiatives : Develops and presents educational materials for cross-departmental in-services and training sessions.
- May perform additional duties as assigned.
Additional Requirements :
- Preferred Qualifications : Bilingual (Spanish) preferred.
- Experience : Two years of experience in developing, implementing, and coordinating programs or projects.
- Education : Bachelor's degree in nursing, health, education, social work, public administration, or a related field (or 4 years of experience in lieu of a degree).
