Position Summary

As a Clinical Support Coordinator with CVS Health, you will assist the Clinical Nurse Specialists with non-clinical duties related to coordinating home nursing for patients receiving specialty medications for chronic disorders in the home.

Responsibilities include, but are not limited to :

  • Coordinate home nursing for assigned specialty patients under the direction of a Nurse Specialist or team supervisor.
  • Identify nursing agency based on established guidelines, provide the nursing agency with clinical documents, nursing authorization and clinical competency link / test.
  • Management of information in multiple EMR systems, that includes but not limited to, documenting patient demographics, provider details, referral process / status note, preparing and sending forms for signature and discharging from system.
  • Assist the clinical nurse specialists with monthly nursing agency authorization renewals.
  • Follow-up with agencies to retrieve documents (competencies, signed nursing service agreements, license / certificate of insurance) and upload to CNS.
  • Assist Nurse Specialists with Medicare / Medicaid agency searches.
  • Complete OIG / SAM searches to ensure agency compliance with federal programs.
  • Patient outreach to provide manufacturer injection training contact information.
  • Perform other duties as assigned.

This is a full-time remote position. In person attendance to training and meetings may be required in the CVS Specialty Pharmacy Branch

Required Qualifications

  • Minimum one year of related experience; administrative support functions, and customer service.
  • Experience as a nursing assistant, pharmacy technician, home patient representative, or customer service representative preferred.
  • Effective verbal and written communications skills.
  • Excellent organization and time management skills.
  • Ability to learn and work with multiple computer applications.
  • Proficiency with Microsoft business software (Outlook, Word, Excel).

Preferred Qualifications

Previous experience working in a specialty pharmacy, medical office or healthcare setting.

  • Knowledge of medical terminology.
  • Proficiency in using electronic health record (EHR) systems.

Education

High school diploma required; some college preferred.

Pay Range

The typical pay range for this role is :

$17.00 - $28.45

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.

This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
