What You'll Do :
As an EHS Case Manager, you'll play a crucial role in our Early Head Start program, focusing on the social and cognitive development of children ages 0-3.
You'll conduct in-person home visits to provide curriculum-based education, health guidance, and connect families with vital community resources.
Your work will support pregnant women and parents of infants and toddlers, helping them navigate their health, social, and educational needs.
Key Responsibilities :
- Plan and conduct weekly home visits to promote child development and family well-being.
- Provide education on prenatal development and early childhood milestones.
- Connect families to local resources and social services, assisting with goal-setting and transportation as needed.
- Maintain accurate client records, ensuring confidentiality and adherence to EHS Performance Standards.
- Participate in community outreach, health promotion, and MCC events to support our mission.
What You Bring : Required
Required
- Associate's degree in Early Childhood Education, Social Work, Public Health, Sociology or Psychology or an Infant / Toddler / Home Visiting CDA credential
- Valid driver's license with a clean driving record.
- Strong interpersonal skills, cultural sensitivity, and a passion for making a difference in the lives of families.
Desired Experience, Skills and Education
- BA / BS degree in Early Childhood Education, Social Work, Public Health, Sociology or Psychology
- 2+ years of experience in community outreach
- Knowledge of child development, pregnancy, breastfeeding, and maternal health issues.
