Registered Nurse (RN) Case Manager - Home Care : Enjoy autonomy and flexibility visiting patients in their homes.
Hours : Full-time Days Monday - Friday Holiday rotation (approx. two holidays per year) Weekend rotation may apply based on staffing levels.
Territory : South East Twin Cities Metro area.

Requirements :
- Graduate of an accredited nursing school with current State of MN RN license
- Two years nursing experience, one year case management experience preferred
- CPR Certification
Responsibilities :
- Perform assessments, develop working relationships with discharge planners, and assess employee skills to client needs
- Visit in-home clients, participate in ongoing interdisciplinary patient assessments
