Job Summary :
A Licensed Social Worker (LSW) who plans, manages, coordinates, implements, and evaluates the ongoing care and psychosocial needs of the patient.
Essential Job Functions :
- The LSW case manager is responsible for ongoing monitoring and evaluation of patient outcomes and for the provision of quality, cost- effective and medically appropriate services.
- Collaborates and assists members of the health care team, the patient and the family to determine home needs for effective, efficient, and appropriate discharge care
- On-call rotation for discharge planning after 5 pm and on weekends

Qualifications
Minimum Qualifications :
- Social Worker licensed in the State of Texas.
- Bachelor or Master's degree required.
- 2 years clinical experience in hospital setting.
- Prefer Utilization Review / Discharge Planning background.
