Job Description : n nPosition Summary : Responsible for conducting possible group as well as individual therapy, documenting client status and response to treatment and coordinating program services with other providers.

Works as a member of a multi-disciplinary team. Prior healthcare experience a plus. BILINGUAL A PLUS ( Can be Spanish / English or Farsi / English candidate, must be fluent and biliterate in languages.

A Behavioral Health Professional provides professional level mental health assessment and counseling services to patients as part of the integrated health team at Wesley Center / Golden Gate location assigned by management.

In this role the incumbent coordinates mental health care with the medical team and support staff, and promotes ongoing collaboration and cohesiveness between medicine and behavioral health care to ensure delivery of whole health care to patients, at a location assigned by management.

Dependent upon the service assignment, the Behavioral Health Professional would be supporting a patient population who are homeless, depressed, anxious, uneducated, chronic pain, stressed, domestic violence, abuse victim, substance abuse, and dying.

The person in this position will work under the direction of the Integrated Behavioral Health Director as a member of primary care team in providing services to patients in the medical clinics as part of an integrated model.

Interventions will be brief, understandable, effective, and consistent with a population-based model of care. Essential Duties and Responsibilities : To perform this job successfully, an individual must be able to perform the following satisfactorily;

Provide individual, group and / or family counseling in office (integrated medical and behavioral health) May be utilized to provide initial assessments and case management services.

Develop individualized service plans and crisis and safety plans. Evaluate and continually monitor client / family progress in meeting goals and anticipated outcomes.

Assist or perform duties related to providing and / or monitoring self-administration of medication. Provide varied verbal and physical crisis intervention and counseling.

Directly responsible to Clinical Supervisor for administrative and clinical issues regarding admission, discharge and utilization management issues.

Maintain clinical records in accordance with all local, state and federal laws and procedures. Update complete staffing notes for multi-disciplinary clinical team reviews.

Receive clinical supervision as required by state law and procedures Directly responsible to the Director of Behavioral Health for administrative and clinical issues regarding admission, discharge and treatment issues.

Candidate will demonstrate a high level of compassion and empathy for each individual that is seen and the expectation is that the candidate will also work within a team setting requiring feedback and consultation with other team members.

Participate in implementing and enforcing safety performance standards. Maintain outcome based reporting spreadsheets and reports that monitor client progress.

Assist with responsibility for appearance, cleanliness and maintenance of facility and equipment. Oversees delivery of appropriate clinical services and ensures direct reports comply with established guidelines and requirements set forth in the ADHS Provider Manual Mercy Maricopa Integrated Care Services Edition, Bureau of Medical Facilities Licensure, and other documents or entities specified by contract.

Uses qualitative and quantitative outcomes and processes data to initiate training, mentoring, or performance improvement activities to assist individuals and / or clinical teams to deliver excellent behavioral health care.

Communicates operational barriers, Adheres to minimum productivity and documentation requirements. Ensures confidentiality of verbal and written recipient information, in accordance with HIPPA standards and Other Duties : Provide quality clinical consultation addressing patient psycho-social concerns in collaboration with the integrated primary care team.

The BHP will be capable of assessment and interventions for patients of all ages regarding psycho-social concerns that may be related to life stressors, acute and / or chronic medical / mental health conditions, motivational barriers to healthy lifestyle, and / or other identifiable behavioral health concerns, while making appropriate recommendations to medical providers and patients The BHP will be capable of assessing patient’s mental health related conditions utilizing DSM-IV-R, PHQ-9, and other relevant diagnostics for the purposes of integrated primary care treatment planning and referrals to community based specialized mental health providers.

Provide brief, health and behavior focused assessments and interventions with active development of behavioral change planning that can improve psychosocial functioning and impact patient’s bio-medical health outcomes.

Cultivate education and self-management skills to targeted populations with comorbid medical and mental health conditions, chronic medical disease, behavioral health concerns, and other health related conditions that impact biopsychosocial health and functioning.

Utilize a variety of intervention approaches which may include but not be limited to the following : Motivational Interviewing, Solution focused therapy, Cognitive Behavioral Therapy, Mindfulness and acceptance strategies, psycho-education, and other therapeutic approaches applied within the integrated primary care behavioral health consultation model all within the integrated medical and behavioral health setting.

Provide education to the primary care team regarding the latest information and knowledge pertaining to integrated behavioral health assessment and intervention, related behavioral health conditions, safety and risk assessment, in order to improve patient care and assist with enhancing primary care utilization of Behavioral Health Consultation.

Establish and build collaborative relationships with medical provider's through shadowing provider Medical visits, familiarization with provider patient panels, provider-directed psychosocial screenings, integrated medical visits, screening patient prior to medical visit and other best practice methods helpful.

Promote smooth interface between medicine and behavioral health care throughout the organization through collaborative relationship building, efficiency with patient flow and "warm hand-offs", accessibility to medical providers, brief consultations with providers and staff, and adherence to scope of practice limitations given the population based approach.

Facilitate referrals to specialized mental health providers and psychiatric services as needed and by utilizing case manager;

will provide on-sit crisis assessment, triage and intervention while coordinating with community based mental health organizations, local hospital, and public safety officials.

Collaborate and co-manage the patient's behavioral health care with consistent, expeditious feedback to referring providers.

The BHC will document in the patient record health and behavioral assessments and intervetions and community based referrals to specialty mental health providers.

Responsible for maintaining accurate electronic patient records while remaining in compliance with all HIPAA and Confidentiality policies and procedures.

Maintain patient charts and records according to established Electronic Medical Records guidelines and procedures for integrated behavioral health consultation model of care.

Protecting and maintain the confidentiality of patient records and information, proprietary information and personnel information.

Completing all clinical documentation clearly thoroughly, accurately and within timeframes specified by Wesley policies and procedures.

Develop treatment plans appropriate for primary care behavioral health patients identifying reasonable, attainable goals and objectives.

Present treatment options and working with the patient to determine applicable goals, objectives and timelines. Provide guidance to the patient to ensure treatment modes and duration of treatment is appropriate for success.

Education / Experience : A Master’s Degree in clinical (professional) counseling, social work, or behavioral health discipline from an accredited college or university.

A minimum of three to five (3-5) years’ of providing behavioral health services (i.e. assessment, diagnosis, or treatment of an individual’s behavioral health needs) in the behavioral health field.

However, if the right candidate applies, fewer years of experience may be considered. Master's degree in counseling, social work and licensed by the Arizona Board of Behavioral Health Examiners as LPC OR LCSW.

Minimum of 2 years clinical experience working with individuals with serious and persistent mental illness. Two years of supervisory experience preferred Due to state licensing requirements, applicants must be a minimum 21 years of age.

Language Ability : Read, analyze and interpret scientific and technical journals, financial reports and legal documents.

Respond to inquiries or complaints from customers, agencies or members of the business community. Write speeches and articles for publication.

Mathematical Ability : Work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry.

Apply concepts such as fractions, percentages, ratios and proportions to practical situations. Reasoning Ability : Solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills : Electronic Health Record (EHR), MS Office, Excel, Powerpoint, Internet Explorer.

Certificates and Licenses : A current Arizona Board of Behavioral Health Examiners License in good standing, or licensed within 6 months of hire, at the independent level in any one or more of the following : Licensed Clinical Social Worker (LCSW);

LPC A current Cardiopulmonary Resuscitation (CPR) Certificate from an approved and accredited training program. Possess a valid driver’s license.
