This is an onsite position.
Position Summary : This position is responsible for supporting the provider-patient relationship, assisting patients in the primary care setting to assure effective communication and care, and for providing direct care and case management to ensure effective communication and care of service.
Essential Duties and Responsibilities :
- Maintains working knowledge of and adhere to the program Manual, agency handbook and Policies and Procedures
- Commitment to providing cost conscious, fiscally responsible, timely, efficient, effective, safe, patient-centered, culturally competent, equitable, and participatory health care.
- Attends all required training's as per Agency Policy, Program Manual and Funder Guidelines.
- Arrives on time for work and remains at work for whole shift and have regular and predicable attendance.
- Maintains appropriate boundaries and ethical standards with staff, clients, visitors and outside providers as detailed in Agency Policy.
- Maintains positive and professional working relationship with co-workers, managers, clients, visitors and outside providers
- Strong and clear written and verbal communication skills
- Excellent computer skills (included but not limited to Microsoft Word, Excel and Outlook)

Program Specific Essential Duties and Responsibilities :
- Interprets for patients without adding, omitting or editing what the patient or provider has communicated.
- Assists with walk-ins, pt. triage, registration, referrals and all other aspects of patient visit as needed.
- Helps providers to understand cultural differences, verbal and nonverbal, to facilitate communication between provider and patient and to assure quality health care.
- Accurately translates medical information in writing for health center staff.
- Assesses patients’ needs and provides advocacy information and referrals to other community agencies as needed.
- Completes patient care documentation or other health center required reports in an accurate and timely manner.
- Participates in PI projects relevant to case management and interpreter services duties.
- Any and all other duties as assigned.
Qualifications and Education Requirements :
One of the following combinations of education and employment experience must be met in order to be considered for the position :
Education and Experience :
Minimum of two years college or Associates degree in a related field required.
Bachelor’s degree strongly preferred.
Medical Interpreter training required.
Two years relevant work or volunteer experience required.
- Certification as Medical Interpreter required or must earn within 6 months of date of hire.
- Bilingual in English and another language appropriate to FHCW patient population required.
- Must have adequate reading / writing skills, including medical terminology, in both languages
