Patient Relations Specialist Requirements & Responsibilities :
We are a reputable Home Health Care agency currently interviewing for an experienced Patient Relations Specialist for our Brooklyn Office.
Key Responsibilities :
- Act as a liaison between patients and aides, ensuring effective communication and service delivery.
- Address patient concerns and inquiries promptly and professionally.
- Coordinate patient care services, ensuring all needs are met efficiently.
- Supervise and support home health aides, providing guidance and resolving any issues that arise.
- Manage patient records and scheduling using HHA Exchange software.
- Maintain detailed and accurate documentation of all interactions and services provided.
- Collaborate with the healthcare team to improve patient care services and outcomes.
Requirements :
- At least 1 year of experience working in healthcare or home care is preferred.
- Bilingual in English and another language is required.
- Proficient in Microsoft Word and Excel.
- Proficiency in HHA Exchange software for scheduling.
- Strong multitasking and time management skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as a collaborative partner.
- Self-driven with the ability to manage projects by establishing objectives.
- Superior customer service skills.
- Excellent written and verbal communication.
- Detail-oriented with strong organizational skills.
