Care Manager
Pay Rate : $19.95
What will you do? As a Care Manager at Aspire Health Partners, your job purpose is to respond, de-escalate, provide peer support, and directly connect individuals experiencing mental health related crisis in our community to the services they need.
This position will work in teams of two (one licensed clinician and one crisis support case manager) to be available for immediate response to dispatch of appropriate mental health-related calls.
Essential functions include :
- Respond both via telehealth and face-to-face in the community to identified mental health crisis calls.
- Utilize assessment and intervention techniques, to provide short term crisis support / counseling and to connect the client to appropriate Aspire and / or Central Florida community resources.
- Formulating an appropriate clinical disposition for the client in need of services.
- Additional internal functions include participation in staff meetings, interdepartmental communication and consultation, and community engagement with stakeholders and resources.
- Operational duties, such as :
- Generating additional documentation, data collection, reports, and diagnostics when necessary
- Collaborating with outcome evaluation processes and / or any other tasks requested by supervisor.
Qualifications
- Minimum Bachelor's Degree in the Human Services field and experience working with adults with mental health or substance use disorders.
- Pass a Level II Background clearance
- Pass a Motor Vehicle Records review
