Interim MDS Coordinator Skilled Nursing Facility

LeaderStat has a new opportunity for an MDS Coordinator (RN or LPN) in Springfield, Illinois. We’re looking for a driven, experienced MDS nurse who can commit to a 6-week contract in a skilled nursing facility.

When you join the LeaderStat team, you’ll be connected with a dedicated specialist before and during your assignment. They will work with you to coordinate onboarding, travel arrangements, and assist with any questions along the way.

Job Duties

• Assessing resident needs and coordinating individualized care plans based on assessment results
• Collaborate with all departments to ensure that care plan guidelines are followed
• Ability to accurately determine PDPM categories, HIPPS codes
• Ensuring that the nursing facility is receiving accurate and maximum reimbursement from Medicare and Medicaid

Qualifications

• Successful completion of an MDS training program
• In-depth working knowledge of RAI user manual, as well as PDPM / HIPPS
• Active Registered Nurse (RN) license in the state of Illinois
• Knowledge of current state, federal, and Centers for Medicare & Medicaid Services (CMS) regulations
• Exceptional attention to detail
• Strong computer literacy and competency skills
• Up-to-date clinical compliance
• Basic Life Support (BLS) or CPR via American Heart Association (AHA)
• Two-step TB test
• Physical exam within the last 12 months
• Successful completion of MDS Assessment via Relias
