A nonprofit organization in New York City is currently seeking a licensed Registered Nurse (RN) for a promising opportunity with their team in Manhattan.
In this role, the Registered Nurse (RN) will be responsible for assisting clients in transition from hospitals back to the community.
About the Opportunity :
- Start Date : ASAP
- Schedule : Part Time Monday, Thurs, Fri
- Hours : 9am to 5pm
- Caseload : 5 to 7 per day
Responsibilities :
The Registered Nurse (RN) will :
- Assess patient health problems and needs reflective of holistic needs, develop and implement nursing care plans, and maintain medical records
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate care plans
- Advise patients on health maintenance and disease prevention including effects of diet and physical activity and provide case management
- Monitor, record, and report symptoms or changes in patients' conditions and record medical information and vital signs
- Provide information to other clinical staff, supportive housing staff, and administrators on nursing standards of practice and procedures
- Provide health education / teaching for patients and their families for specific medical needs, preventative care, and how to access and utilize healthcare services in the community
- Conduct counseling sessions, with direction as needed by other qualified professionals, according to principles of communication, human behavior and psychiatric nursing
- Participate in risk management and quality assurance concerning health related issues
- Provide emergency care and crisis intervention, as needed
- Handle medication reconciliation and observe and records effects and monitor for side effects
- Perform other related duties, as assigned
Qualifications :
- 1+ year of Nursing experience working with individuals with Mental Health needs
- Associate's and / or Bachelor's Degree in Nursing
- NYS Registered Nurse (RN)
- Knowledge of professional Nursing concepts, principles and practices
- Knowledge of concepts and principles from the Biological Sciences that are supportive of nursing practice
- Knowledge of concepts and principles of Health Promotion and Disease Prevention
- Knowledge of Emergency treatments and interventions
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
