What We Are Looking For
The Supervisor, CMT Behavioral Health is responsible for supervision of the day-to-day operational and clinical oversight of Behavioral Health staff in Care Management.
The Supervisor will ensure positive patient experiences, effective coordination, efficient operations, and accurate data collection / reporting.
Clinical supervision of ACSW's working within CMT. Crisis management for CMT and clinical staff / providers. Will participate in continuous process improvement practices and ensure adherence to policies and standards.
Will provide training and development to CMT BH staff. Responsible for time entry, performance measures, and staff disciplinary processes.
In collaboration with management, responsible for the operationalization of program requirements. Position may act in the capacity of a therapist based on program needs, including individual, group, family therapy, and / or brief intervention.
This position will require frequent travel between clinical sites and patient-oriented locations.
Schedule : 5 days per week, 8 hours per day, Monday - Friday, 7 : 30 am-4 : 00 pm Location : San Bernardino, CA
This is not a remote position.
ESSENTIAL FUNCTIONS AND DELIVERABLES
- Responsible for compliance with program and contractual requirements.
- Clinical supervision of ACSWs and BH staff in CMT.
- Day-to-day operational oversight and supervision of CMT BH staff.
- Assist with internal audits. Provide feedback to staff on program guidelines and workflows.
- Assist in developing and managing outreach, community engagement, and patient care plans.
- Will oversee Crisis Management and assist the department as necessary.
- Ensuring CMT BH staff meet productivity, enrollment, and caseload expectations.
- Patient risk assessments and BH follow-up. Ensure patient risk assessments are complete and adequate follow-up and guidance are provided.
Provide recommendations, treatment resources, and referrals for CMT BH Staff.
- Co-sign documentation for CMT BH Staff. Ensure documentation meets program requirements.
- Daily : Attend huddles, provide feedback and guidance on patient care and treatment plans, and maintain up-to-date knowledge of mental health and community resources necessary for patient linkage.

QUALIFICATIONS :
- Education : A Master's degree in Social Work (MSW) required.
- Licensure / Certification : Active and unrestricted CA Licensed Clinical Social Worker (LCSW) required. Current CPR / BLS certification (must be American Heart Association or Red Cross accredited program), valid California driver's license, and auto insurance.
As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
Experience : 3 years working as an LCSW providing therapy services required. 1 year of clinical supervisory experience preferred.
Previous experience working in crisis management or county-level populations preferred.
Essential Technical / Motor Skills : Must be proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook). Must be able to use widely support internet browsers.
Must have the ability to use variations of electronic health records and other various databases.

Essential Mental Abilities : Must be flexible to perform a variety of tasks. Must be well organized and a self-starter.
Must have strong analytical and problem-solving skills.
Work Eligibility : Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
