POSITION TITLE : Rehab Clinician
REPORTS TO : Case Manager Supervisor
POSITION STATUS : Full-Time and Part-Time
Role :
Under the general direction of the Case manager supervisor, the Rehab Clinician provides CANS identified Skills Training Interventions.
Services include individual client education, and / or prosocial skills, or LAR skills training. Collaborates with the client and a multidisciplinary treatment team for treatment plan development and implementation.
Position requires completion of agency-approved clinical documentation in an electronic medical record and familiarity with the Texas Administrative Code.
Job Duties and Responsibilities :
Provide Skills Training for up to 6 clients with a minimum of 27 Face to Face hours weekly.
Meet with Case Managers for supervision.
Implement Treatment Plan with Clients- provide one-to one CANS identified Skills training interventions. Meet at school and in-home weekly per the recovery plan frequency requirement.
Cultivate relationships with families.
Work with school staff (or agency staff) as needed.
Adhere to the following KPI’s and ensure Rehab Clinician compliance : 95% of notes submitted within 48 hours.
95% of notes created utilizing check in / check out process 50% percentage of community-based visits (child).
Other duties as assigned.
Requirements :
Demonstrated competency in the provision and documentation of mental health rehabilitative services.
Bachelor’s degree in social service
High school diploma

INITIAL APPROVAL & COMPETENCY
The above employee has demonstrated the capability to satisfactorily perform the responsibilities of this position. All Positions are on a 90- day probationary period at which point employee will be re-evaluated to document training has been effective and competency approved.
