SUMMARY OF DUTIES AND RESPONSIBILITIES :

Conduct screenings and perform intakes. Link guests to resources for income, primary care, substance use treatment, mental health treatment, employment, clothing, housing, school, and any other resources guests need to successfully re-enter into the community.

Work with Re-Entry Program Team; Program Manager, Employment Specialist, Peer Specialist, Overnight Monitors to coordinate care.

PRIMARY DUTIES AND RESPONSIBILITIES :

  • Facilitates and documents group and individual one-on-one sessions.
  • Ensures participant conformance with program rules and regulations, department policies.
  • Responsible for conducting assessments, creating, and maintaining service plans, short-and long-term goals, and case management for a caseload of 15-20 guests.
  • Facilitates twice a month one-on-one session with the guest to help maintain their service goals.
  • Maintains accurate records by entering documentation of all assessment, services, crisis management, and discharge services into various electronic systems for all guests in compliance with ECS practices.
  • Coordinates transition plans and completes the discharge from the program.
  • Maintains client data and case notes in all relevant systems and supports reporting of all relevant data to funders as needed.
  • Be able to facilitate learning sessions one-one or in small groups for : Life Skills, Anger Management, Socialization, Release & Reintegration.
  • Identifies the need for other services and benefits (including literacy, computer competencies, ESL courses, high school diploma or equivalency education, or postsecondary education opportunities).
  • Makes referrals for other support services (including housing, legal, transportation, benefits counseling, childcare, substance abuse counseling).
  • Provides light-touch financial coaching and referrals for more intensive financial assistance as available, to mitigate barriers to employment and financial security.
  • Be able to quickly assess and respond to a variety of situations, including medical emergencies, conflict resolution and intervention, and other tenant emergencies.
  • Possess a working knowledge of crisis intervention techniques and be able to implement them.
  • Communicates with Program Managers, Employment Specialist, Peer Specialist, Overnight Monitors.
  • Performs other duties as assigned.

Qualifications

REQUIRED QUALIFICATIONS :

High School Diploma / GED required. Experience working with individuals who have been incarcerated or in a related human service field, plus one (1) year progressively responsible, paid experience as a case manager, counselor, or client advocate working with low-income individuals who have justice system involvement, mental health needs and histories of substance use.

Experience preferred who have worked in the justice system or to homeless adults. Must have strong administrative and organization skills, Computer skills are required.

Strong interpersonal skills and the ability to communicate effectively both orally and in writing are essential.

OTHER REQUIREMENTS :

Must have CPR / First Aid. Must secure finger image screening (Live Scan), annual TB screening and provide proof of COVID vaccination and booster.

Must be able to pass CDCR background check. Must be able to meet physical requirements of the position which may include long periods at a desk and / or computer workstation as well as the ability to stand, sit, kneel, reach, bend and able to lift 30 pounds.
