HomeWell Cares Services is a rapidly growing home care agency. We are looking for a motivated individual to join us as a Registered Nurse / Care Manager.
The Registered Nurse / Care Manager is responsible for coordinating, implementing and supervising the delivery of services to agency clients in alignment with the company’s mission and standards of care.
This position is accountable for adherence to regulatory and agency requirements for service provision and ensures compliance with and maintenance of client records.
The Registered Nurse / Care Manager is also responsible for providing and overseeing ongoing training and education to staff in the client’s home, online and / or in the agency office.
Responsibilities and Duties
- Evaluate referred clients for appropriateness of admission and coordinate the clinical intake and business forms necessary to onboard new clients.
- Develop and implement a written care plan for each client with input and approval by the client and / or designated client representative.
- Supervise client care provided by agency caregivers.
- Regularly communicate with the client and / or designated client representative to follow-up on service delivery and ensure client satisfaction.
- Perform scheduled and non-scheduled in-home client support visits.
- Conduct reassessments according to the agency’s established frequency and as needed due to a change in client condition or recent event.
- Participate in the coordination of services when more than one community service is providing care to a client such as another home care agency, home health agency, hospice agency or facility.
- Serve as a resource for clients and families, facilitating client education, empowerment, and quality of life; provide recommendations and referrals as needed to third party services.
- Ensure client records are current and properly maintained in accordance with agency policies and procedures.
- Provide needed training and education to staff in the client’s home, via telephone and / or in the agency office.
- Assist with the development and revision of client care policies and procedures.
- Participate in personnel evaluations for caregivers.
Qualifications and Skills
- Must be a licensed Registered Nurse in the State of Florida; a Bachelor’s Degree in Nursing strongly desired.
- Preference will be given to candidates having demonstrated experience using WellSky / ClearCare
- Required 1+ year of experience as a Registered Nurse in a home health care setting.
- Experience with Microsoft Office, sound computer skills, and strong organizational abilities. Excellent interpersonal relations abilities.
Excellent telephone skills.
- Able to work independently, demonstrating sound judgment.
- Read, write, speak, and understand English as needed for the job. Fluency in a second language is a benefit.
- Valid driver’s license and reliable vehicle with valid insurance
- Must have an active State of Florida Level 2 Background Check or pass one
- Be available as required for on-call duty outside of normal office hours.
