Client location and schedule hours :

Client is located in the Brentwood area, schedule is M-T-W 9 : 00 am to 1 : 00 pm

Job Responsibilities :

Social History : Client lives alone with no pets and has been a widow since 2021.

Diagnoses : Osteoarthritis, Osteoporosis, Cracked Pelvis (recovering), Aphasia, Hypothyroidism, GE Reflux, Fall Risk Allergies : Meds-Pyridium, Allopurinol, Bactrim Food- None

Goals & Objectives : Client needs someone to prepare and serve breakfast, clean kitchen, once per week do laundry / bed linens.

She needs help into and out of the shower, Light housekeeping. Observe on transfers to assist if needed. Offer / remind fluids for hydration.

May run errands or take to appts as needed. Prepare something for her lunch at end of shift just prior to leaving. Remind her to take her meds set up by her daughter.

We need someone who has a lot of Dementia experience.

ADL's :

Mobility : Client was observed to use a rollator in the house and sometimes walks short distances without it. She can get up and down from a seated position with the help of her lift chair.

She is a fall risk, so should be observed for imbalance or exhaustion and encouraged to rest. She needs help into & out of the shower as well as in and out of a vehicle.

Bathing / Dressing / Grooming : Client needs help putting on tops due to pain and sensitivity in her left shoulder that she broke in February 2024.

She may need help putting on pants as well. Client needs help getting into and out of the shower and for CG to stand by for more assistance such as drying off.

She may need help with her hair (wig), but CG should not mention her hair until client tells her what she would like. Client can brush her own teeth and can pick out her own clothes.

Eating : Client can feed herself but has trouble standing up for long periods to cook for herself, so needs CG to prepare and serve her breakfast and lunch.

She is right handed and weak in her left. Client has been ordering groceries from Publix but may let the CG pick up groceries or help put them away and help create her lists.

Communication : Client is clear and articulate in her speech and engages well. She does not have a hearing loss. She is well-mannered and has a sweet personality.

She reports that although she has not been diagnosed with dementia, she is frustrated that she often forgets recent events and needs reminders.

Qualifications :

Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member

Ability to effectively communicate through written and verbal language

Valid Driver's License and adequate auto insurance

Consent to criminal, elder abuse, motor vehicle and sex offender background checks

Consent to drug and alcohol testing
