Summit Home Care is a family-owned and operated home care agency. We are currently looking for compassionate and reliable per diem Clinical Supervisors to join our team of dedicated professionals.
The clinical supervisor is responsible for the oversight of their patient from admission through discharge.
Responsibilities :
Works with Patient Coordinator to ensure accurate clinical documentation for case opening
Opens cases and initiates the plan of care, medical orders, MAR, and TAR and revises as necessary
Conducts monthly client visits in-home and virtually as needed
Is the liaison between Summit and community, clients, facilities, etc.
Evaluates the outcome of care while providing support to ensure the services being rendered are meeting the client’s need
Informs the physician and other staff of changes in the client’s needs.

Requirements :
- Current RN license in the state
- Willingness to travel
- 2+ years experience supervising
- Previous Home Care experience
- Knowledgeable with NJ DOH regulations and policies
- Up to date first aid and CPR certification
- Exceptional staff training and customer service skills
- Compassionate and friendly demeanor
