As a Nurse Practitioner, you’ll contribute to our success in the following ways :
- Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
- Prevents unwanted hospitalizations by meeting patient and family needs in the home.
- Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
- Compliance with applicable laws and regulations with respect to Collaborative Agreements.
- Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation.
- Provides training and continuing education for staff.
- Assists in development of clinical practice guidelines / standards in support of quality palliative care.
- Adheres to the practice of confidentiality regarding patients, families, staff and the organization.
- Collaborates / Communicates with Medical Director, attending physician, branch leadership and IDG.
- Performs other duties / projects as assigned.
What is Required?
- Masters Degree in Nursing and certification through a national Nurse Practitioner Licensing board.
- 3+ years clinical nursing experience
- 1+ year post-graduate clinical nursing experience in the hospice and palliative care field.
- Prior experience in Family Medicine, Geriatrics and psychiatry preferred.
- Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.
- Board Certification in Palliative Care Preferred.
- Active DEA license.
- Able and willing to travel up to 50% as needed to support business and patient needs.
