Job Summary
The Care Coach is a nationally certified medical assistant who works directly with patients in one-to-one encounters and group sessions to reinforce self-management of their chronic condition while close monitoring their established care plan goals.
Essential Job Functions

- Promote Patient Care Plan goals and reinforce lifestyle modification.
- Conducts patient face to face visits and group workshops within their assigned center.
- Ensure that patients with Chronic Conditions are being seen in a timely manner and in their preferred mode of learning.

- Care Gap Closure - Collaborates with the Center to ensure patients complete their applicable preventive care gaps.
- Promote established care plan goals and reinforce lifestyle modification education.
- Interdisciplinary Care Team - Participates in center team meetings including huddles, PODs and direct communication with providers and the care team.
- Works with the Care Educator RN and the Care Team to collaboratively discuss complex patient cases.
- Documentation - Accurately documents patient information into the EMR system.
- Ensure all Health Coaching activities with patients are accurately documented within our population health platform.
- Health Promotion - Participates in health fairs and health promotion events.
- Ensure there is a Care Programs presence at both Santias and FL Blue events.
- Build trusting relationships with patients. Identifies and meets with same day patients who will benefit from the care education program.
- Provide coaching services at the POC.
- Complete a care plan for each patient enrolled in the Care Program.
- Create a plan of care for the patient based on the chronic condition, lifestyle changes recommended by the provider and the goals jointly established with the patient.
- Ensure that the patient and the care team are aware of the healthcare goals and the interventions established between the Health Coach and the patient.
Qualifications

Required Education
- High school diploma or equivalent.
- Graduation from a nationally accredited Medical Assistant (MA) program.
Required Experience
Two years Medical Assistant experience in primary care setting is required.
Required Licenses and Certifications
- Active MA national certification.
- Certified Clinical Health Coach (certification required to be obtained within the first 4 months of employment).
- CPR / BLS for Healthcare Providers is required.
Required Knowledge, Skills, and Abilities
- Experience in Microsoft Word, Excel, PP.
- Must always have excellent oral and written communication skills with evidence-based data and etiquette.
- Ability to think on their feet to solve problems for patients while achieving care program team goals.
- Must be detail-oriented to ensure accuracy of reports and data.
- Ability to maintain effective and organized schedules to ensure timely patient flow.
- Ability to manage stressful and fast paced work environments.
- Performs other assigned duties by the Lead Educator RN and / or Center Manager.
Preferred Qualifications
Experience working with chronic disease education management is preferred.
