Description
Center for Human Development (CHD) Meadows Homes provides residential housing supports for individuals with intellectual disabilities.
While supporting and encouraging them to live full active lives. Direct Care professionals provide a supportive residential environment for developmentally disabled adults living in a community-based group home.
They seek to provide an environment that encourages growth and supports individuals to live as independently as possible in all aspects of life.
RESPONSIBILITIES :
Supporting clients' activities of daily living.
o Personal care
Bathing, toileting, grooming, teeth brushing, hair care
o Household chores
Dishes, laundry, shopping, preparing meals based on dining guidelines
o Transportation
Advocating for individuals' within various community settings,
o Maintaining social relationships
o Visiting with friends and family
o Assisting individuals to make new friends
o Outings
Bowling, dances, vacations, religious events
Support individuals with medical needs
o Vital signs
o Medical records
o Medical appointments
o Medication administration (MAP certification)
o Medical devices
Hoyer lift, wheelchair, ostomy, walker, shower chair
o Caring for individuals with complex diagnoses
Autism, dementia, down syndrome, intellectual disability (IDD), cerebral palsy, stroke, hospice, age related mobility decline
Providing opportunities to optimize independence
o Encourage and foster independence while providing necessary supports.
o Develop professional, supportive, and trusting relationships with clients
Manage behavioral needs with compassion
o Positive Behavior Interventions and Supports (PBIS)
o Assisting individuals in de-escalation strategies to help with self-regulation
o Physical intervention (restraints)
o Management of behavioral episodes
Outbursts, aggression (verbal and physical), elopement, anxiety, Depression, self-injurious behavior (SIB)
Documentation
o Daily shift notes
o ABC behavioral data collection (Antecedent, Behavior, Consequence)
o Appointment records
REQUIREMENTS :
- Spanish speaking individuals are encouraged to apply.
- Strong interpersonal skills; empathy for others; demonstrated commitment to serving those with significant needs; ability to work well within a team model and independently.
- Strong communication skills, including verbal and non-verbal
- Knowledge of common behavior interventions
- Driver’s license and Personal vehicle for work use
o When a transportation van is unavailable you will be expected to transport individuals in your own vehicle.
The following certifications are preferred. If you do not have any of these CHD offers training at no charge.
o First Aid and CPR
Hands on training component required
Not accepted certifications (Basic, BLS)
o Medication administration certification (MAP)
MAP certification is required to be completed within 6 months
o Safety Care (QBS)
All candidates must complete and pass background record screening process.
