The Healthy Community Grant Manager will manage a portfolio of community health grants and population health initiatives including, but not limited to violence prevention / intervention and health-related social needs. The Grant Manager, Healthy Communities will manage new and existing grant-funded projects and partnerships within and outside of the health care system. The Grant Manager, Healthy Communities will ensure grant deliverables are completed on time and within the budget; they will actively raise additional funds to pilot, scale, and sustain impactful programs and other tasks, as assigned.
Manage grant-funded projects, budgets, and / or staff from concept, research / data collection, grant writing, contracting, implementation and monitoring, purchasing / expense reconciliation, and reporting. Ensure timely follow-up on assigned projects.
Convene interdisciplinary teams to accomplish grant deliverables, including working with functional leaders, advisors, community partners, funders, and third-party evaluators. Ensure comprehensive data for reporting / quality improvement is tracked and analyzed. Maintains privacy standards of protected health information (PHI).
Lead the submission of new grants to pilot, scale, evaluate, iterate, and sustain community health programs.
Manage training and educational opportunities for staff and project collaborators.
Bachelor's of Science, Public Health / Administration, Social Work, Business Administration, or related field. Project Management Professional (PMP) within 6 months.
