PRIMARY JOB FUNCTIONS
Provide access to a wide range of services for clients of the BPCC as assigned by the coordinator.
Properly assess need and devise individualized case management plan for each client.
Serve as a liaison between the client and various service providers to ensure continuity of care.
Effectively implement written service plans to meet client goals and objectives.
Develop and maintain adequately detailed and accurate client records.
Provide transportation to clients when necessary.
Provide accurate documentation in debriefing for weekly and monthly reports / meetings as stipulated by program demands.
Perform all other duties as assigned by the BPCC Coordinator.
MINIMUM QUALIFICATIONS :
Bachelors degree in social work, psychology or other human service field. Experience working with individuals with serious mental illness providing case management services or other outreach services is preferred.
Minimum of one (1) year experience in social services / human services field required.
Valid Alabama driver's license and a suitable automobile to permit the required travel. Must be insurable and maintain insurability under JBS's automobile insurance carrier's standards.
Must show and maintain car insurance equal to or in excess of the State of Alabama standards for automobile insurance coverage.
Certified as a Case Manager by the Alabama Department of mental health or will attain such certification within a month of employment.
KNOWLEDGE, SKILLS AND ABILITIES :
Experience working with individuals with mental illness and substance use disorders.
Considerable knowledge of the special needs and behavioral characteristics of individuals with mental illness and substance use disorders.
Ability to work autonomously.
Extensive knowledge of the community and available community resources.
Knowledge of the rules and regulations governing the human services provider agencies in the Birmingham community.
Knowledge of the legal and ethical issues relative to confidentiality of client records and the ability to interpret and apply them within the scope of the case management responsibilities.
Demonstrable ability to communicate effectively with a multi-disciplinary team.
Considerable knowledge of casework principles and methods related to case management.
Working knowledge of interviewing techniques and principles.
Ability and willingness to document activities and maintain records.
Skills in interpersonal relationships and dynamics.
Considerable written and expressive communication skills.
Acquainted with the use and side effects of psychotropic medications.
Ability and willingness to work in area shelters, boarding homes or foster homes as well as other sites where adults with serious mental illness may reside.
Demonstrable ability to work independently and to make sound judgments regarding client care.
