Touching Hearts at Home of Southwest Boston is seeking a Client Care Manager / Field Supervisor with a customer service focus to help us continue to ensure that we are providing quality home care for seniors in need.

We are looking for a senior care professional who has case management, social work or clinical liaison experience in either a senior care community or home-based setting.

Purpose :

The Client Care Manager / Field Supervisor will oversee all aspects of client care after the care has started. This means communicating with our caregivers, families and professional referral sources to ensure that care plans are accurate, up to date and being followed.

Client Care Manager / Field Supervisor Key Responsibilities :

  • Be the main point of contact for all day-to-day questions or concerns from caregivers, family members or senior care professionals regarding client-related issues.
  • Demonstrate excellent customer service for both internal and external stakeholders.
  • Ensure quality of care for Touching Hearts clients via random and scheduled home visits and making routine goodwill calls.
  • Build lasting relationships with professional referral sources and family members; provide counsel and input intended for the best interest of the client.
  • Coach and mentor caregivers to improve their skills to better address specific client needs; this includes facilitating skill-based portions of caregiver training.
  • Draft and revise care plans to ensure our care team has the most up-to-date information to better prepare them for their shifts.
  • PLEASE NOTE : This is an in-person position Monday through Friday from 8 : 00 am to 4 : 30 pm; although you will often be on the go visiting clients! There may be client needs that come up after hours or on weekends that may have to be tended to;

it's not a regular occurrence, but it does come up.

Client Care Manager / Field Supervisor Position Requirements :

A successful candidate will have proven case management skills in either home-based or community-based settings. The candidate will also be passionate about working with seniors and experienced in coordinating their care.

  • 3 or more years of experience working in a case management or social work role. Preferably in a senior care community or home care setting.
  • Action Oriented Mindset : We are looking for a doer. Someone who is proactive vs. reactive to address both client, family and caregiver needs.
  • Team Player : This candidate needs to be someone who isn’t afraid to step into a different function or role; someone who isn’t afraid to put in the extra hours in the evening or on a weekend to address an urgent or temporary need.
  • Growth Oriented : We are looking for a candidate who wants to help our business grow and become a better provider and employer.

This candidate also must have a willingness and a desire to develop their own skills.

  • Comprehensive knowledge of local senior care resources to better inform and provide care for our clients.
  • Strong organizational and communications skills both written and verbal.
  • Passion for helping seniors remain safe and in-place wherever they call home.
  • Willingness to fill shifts as a caregiver personally when needed and serve as on-call caregiver.
  • Willingness to take on additional responsibilities in other disciplines as needed.
  • Valid driver’s license and reliable insured vehicle.

Touching Hearts provides in-home care for the elderly non-medical companion care includes services like hygiene assistance, light housekeeping, meal preparation, transportation, social companionship, and safety oversight for clients with dementia or physical challenges.

Our mission is to empower older adults to age in place and avoid hospitalizations.
