Social Worker Full-Time
Sterling Healthcare and Rehabilitation Center, a 164 bed facility, recognizes and respects, that Sterling is home to our residents.

We are searching for experienced, caring, & dependable applicants to join our TEAM.
Responsibilities
- Develop social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
- Experience in short-term and long-term care
- Maintain grievance process and logs to positive resolution
- Participate in Utilization Review and other committees as directed and needed
- Assures that notes are informative and descriptive of services provided and of residents’ responses to services
- Prepares and completes social service documentation in timely manner
- Refer resident / family member to appropriate social service agencies when facility does not provide services or needs of resident
- Develop a social history on each resident
- Completing sections of the MDS
- Coordinating room changes
- Participating in behavior meetings
- Develops discharge plan at the point of admission to provide direction for the resident's stay
Qualifications
- BSW or MSW in Social Worker
- Knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care
- Previous long-term care experience is desirable. Past supervisory experience preferred.
