JOB SUMMARY : Reporting to the Manager of Acute Care Services independently implements the nursing process to provide safe and effective patient care throughout selected / assigned hospital nursing departments ( departments ).
He / she demonstrates the knowledge and skills necessary to provide individualized, age-appropriate care to patients served in these departments.
Provides clinical leadership, educational support, policy development and implementation of evidence based practices in support of Acute Care Services within the hospital.
Will be directed, or autonomously go to, any department for which s / he is trained and competent, at any given time to provide nursing support, including break relief.
Maintain the required proof of competency to ensure they are performing at the expected level of patient care.
PERFORMANCE DIMENSIONS AND TASKS
Essential Function
1. Assesses the needs of patients and documents the identified needs in the patient record.
a. Interviews the patient or other significant source and completes appropriate history information in medical record.
b. Performs physical, psychological, sociocultural, spiritual, economic, and life-style factor assessments and documents findings.
c. Utilizes multiple information sources including family, significant others, and healthcare team members in the assessment.
2. Formulates diagnosis(es) appropriate to the patient’s healthcare needs.
3. Develops a plan for the care of the patient and the desired outcomes to be achieved.
4. Implements care for patients guided by the plan of care.
a. Sets priorities for patient care.
b. Safely performs and documents the nursing procedures required by the patient assessment consistent with evidence-based standards and MMHS policy.
c. Demonstrates proficiency in performing nursing skills required by patient population
d. Demonstrates safe and efficient use of specialized equipment used in the clinical area.
e. Demonstrates safe, accurate administration of medications used in the clinical area.
f. Prioritizes own workload relative to patient care and department needs.
g. Completes clinical assignments in a timely fashion including maintenance of equipment and supplies necessary for provision of patient care through the next shift.

b. Follows Infection Control policies / procedures, i.e., prescribed hand-washing and universal or transmission based precautions.
c. Maintains cost containment efforts by prudent use of supplies and equipment; charging supplies and equipment appropriately.

a. Utilizes the steps of problem solving to resolve identified patient or staff related problems.
b. Reports identified problems using the incident reporting process and chain of command.
c. If available, responds cooperatively to requests for assistance to other departments.

d. Develops or assists in the development of clinical policies and procedures based on current research and guidelines.
e. Participates in or leads the implementation of evidence-based practice projects.
f. Participates in the department’s performance improvement and quality initiatives throughout all project phases.
g. Assists with department recordkeeping as directed by the manager.
h. Facilitates department patient care assignments and assists in staffing projections.
i. Participates in department charge reconciliation and serves as a resource to staff regarding department charges.
j. Participates in the evaluation of products and supplies needed for patient care.
