Job Description
Location : Multiple Clinics in Orange County. Travel between Huntington Beach, Costa Mesa, Newport Beach, Irvine, San Clemente, Laguna Beach, Tustin.
No more the 2 clinics per day.
Responsibilities :
- Provide multidisciplinary patient care from admission through discharge.
- Conduct history and physical exams, diagnose, and treat medical problems.
- Order and interpret diagnostic and laboratory tests.
- Perform minor procedures and facilitate the discharge process.
- Identify and execute Annual Wellness Visits (AWV) and Annual Medicare Advantage Visits (AMAV) for Medicare patients.
- Manage care for patients with chronic illnesses, particularly in the adult and geriatric populations.
- Demonstrate advanced knowledge in Risk Adjustment programs and HCC coding and documentation guidelines.
Skills :

- Competence in relevant technology, tools, and equipment.
- Strong clinical knowledge and understanding of applicable procedures.
- Commitment to patient safety.
- Ability to travel locally between organizational locations. Qualifications :

- Masters degree in Nursing.
- Advanced clinical knowledge and skills in patient care and management.
- Proficiency in Microsoft Office products.
- Current RN / NP license in California, furnishing license, and DEA number.
- BLS and ACLS certification.
- National board certification as a Nurse Practitioner in the area of specialty.
- Preferred :
- Two years of recent clinical experience as a Nurse Practitioner in ambulatory and acute healthcare settings.
- Bilingual or multilingual candidates.
