Location : 2995 Red Hill Ave, Costa Mesa, CA 92626

Duration : 3 Months

Schedule Notes : 3-5 days a week; M-F, 8-5 Multiple Clinics; Orange County locations (Huntington Beach, Costa Mesa, Newport, Irvine, San Clemente, Laguna Beach, Tustin).

NOTES :

Nurse Practitioner would travel to a different clinic daily, no more than 2 clinics per day. Experience with senior wellness visits;

chronic condition management and coding, quality care gap management required. it will be outpatient setting, working alongside primary care providers across all our Orange County clinics.

The NPs will be conducting Annual Health Assessments for senior patients (similar to annual wellness visits).

Duties :

Primary Duties and Responsibilities : The Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Hoag Hospital and its affiliates, coordinating seamless patient care from admission through discharge.

Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process.

Hoag Clinic Primary Care / Internal Medicine : Responsible for identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and Annual Medicare Advantage Visits (AMAV) and execution of guided processes to complete AWV and AMAV.

Works with patients with chronic illnesses commonly encountered in the adult and geriatric patient population. Responsible for assessment, diagnosis, education, health promotion, and care coordination for patients with acute and chronic health needs.

In addition, shows advanced knowledge in Risk Adjustment programs and HCC coding and documentation guidelines.

Travel Position may require local travel between various Organization locations.

Education :

Required : Master’s degree in Nursing. Advanced clinical knowledge and skills in the care and management of patients. Solid computer technical skills required, including proficiency with Microsoft Office products (Outlook, Word, Excel, Powerpoint, and Visio).

Preferred : Two years of recent clinical experience as a Nurse Practitioner, preferably within an ambulatory and acute healthcare environment, related to the care of Internal Medicine or Geriatric patients.

Qualified bilingual or multilingual candidates. Licenses Required : Current license in good standing to practice as a Registered Nurse / Nurse Practitioner in California.

Furnishing license issued by the State of California and personal DEA number.- Certifications Required : BLS and ACLS certification.

National board certification as Nurse Practitioner in area of specialty.
