Bilingual (Spanish / English) Medical Case Manager Job Description

The Bilingual Medical Case Manager position will work in an interdisciplinary team setting, based out of Philadelphia FIGHT's YHEP Pediatric and Adolescent Health Center.

The Bilingual Medical Case Manager will provide long and short-term medical case management services, therapeutic support, treatment adherence and benefits coordination to pediatric and adolescent patients living with HIV and other chronic health conditions.

This includes connecting clients to necessary resources and public benefits such as Medicaid Assistance, childcare CCIS, food assistance (SNAP), housing rebate programs, medication assistance programs, and additional resources such as shelter / housing, food banks, substance treatment programs, behavioral health programs, and other necessary resources that will enhance clients' lives.

Clients may also require advocacy with barriers to their education or specialized school services. The Bilingual Medical Case Manager is also a mandated reporter and would be responsible for assessing and reporting any emotional, physical, or psychological harm to a minor patient.

Qualifications :

  • Literacy and fluency in Spanish and English.
  • Minimum of a Bachelor's degree in Social Work (or related field) from an accredited college; MSW preferred.
  • Knowledge and familiarity with local social services resources including public benefits.
  • Knowledge and familiarity with resources for immigrant populations.
  • Ability and desire to work productively with highly stigmatized and diverse populations using a strength-based framework.
  • Excellent interpersonal skills and ability to work collaboratively with clinicians and patients.
  • Ability to exercise crisis interventions.
  • Experience working with people living with HIV / AIDS and other chronic health conditions highly desired.
  • Ability to work on-site.
  • Ability to work from home with Wi-Fi and personal computer access.

Required Screenings

  • Must complete Philadelphia FIGHT Credentialing and Privileging to provide direct services with Federally Qualified Health Centers.
  • Philadelphia FIGHT requires proof of completed COVID-19 vaccination series for all new employees hired. Medical and religious exemptions will be honored consistent with EEOC regulation.
  • Twenty hours of professional education and training will be required on an annual basis as described by AACO. The AACO core training series must be completed within 12 months of hire.

Responsibilities include :

Clinical Responsibilities :

  • Serve clients engaged in medical care at YHEP Pediatric and Adolescent Center in clinic or by telehealth follow-up.
  • Maintain contact with clients based on program need, client need, and service care goals.
  • Provide short term case management interventions connecting clients or their families to necessary resources such as shelter referrals, utility assistance programs, food banks, clothing banks, substance treatment programs, behavioral health programs, gender affirming care, immigrant programs, and other necessary resources.
  • Identify appropriate resources and external support systems to address client needs including advocating and negotiating linkages to resources.
  • Assess and advocate for client's potential eligibility for Medicaid, Emergency Medicaid or CHIP. Assist with connecting uninsurable clients to necessary resources to enhance their medical care.
  • Provide trauma-informed therapeutic support in collaboration with interdisciplinary team, including crisis situations.

Other Responsibilities :

  • Maintain accurate and timely documentation of all client contacts (using D.A.P. format), forms, and applications in EHR software.
  • Provide cross-coverage with other Medical Case Managers and health centers.
  • Maintain confidentiality standards in accordance with applicable laws and regulations, specifically including the Health Insurance Portability and Accountability Act (HIPAA) and the Confidentiality of HIV Related Information Act (Act 148).
  • Contribute as a team member and share in the responsibilities required to maintain operations and serve the mission of Philadelphia FIGHT.

This includes, but is not limited to, obtaining training, participating in events, flexibility in scheduling, covering the commitments of coworkers when they are unable, and other duties as assigned.

Mandated Reporting for minor patients where harm is perceived or evident.

Other : Initial probationary period is three months.

Salary range : $53,000 to $56,000 and competitive benefits package.

Work environment : Generally, works in a clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting.

Moderate noise (examples : business office with computers and printers, light traffic). Hybrid office primary / work from home option for admin.

Work hours : 40 Hours / Week (M-F 9 : 00am - 5 : 00pm). Availability for occasional work outside of regular office hours.
