Description :
This individual will provide case management, support and advocacy while working with families and individuals in need who seek out assistance and services through emergency stabilization and homeless prevention.
Requirements :
Requirements include a Bachelor’s Degree in social work, human services or a related field with experience in case management, working with families and individuals in need or a high school diploma or its educational equivalent with at least 3 years of case management experience in the previously stated fields.
Also required is a solid understanding of case management and / or social work protocols and practices, and community resources.
