Responsibilities : Coordinating with referral agencies in facilitating step down / direct transfer admissions to both our inpatient and / or outpatient continuum of care.
Facilitating all aspects of the assessment and admission process for patients seeking evaluation for psychiatric (with possible co-occurring substance abuse) treatment.
Provide a comprehensive level of care assessment for providing a recommendation for treatment within our continuum of care.
Provide efficient and effective handling of inquiry calls from referral sources, patients, and family members seeking treatment options.
Coordinating with insurance companies for authorizations of recommended treatment Responsible for accurate and timely documentation.
Works collaboratively with the department Director and Supervisor to ensure compliance with state and facility requirements.
For Nurses : provide comprehensive medical evaluations for referrals, conduct nurse to nurse reports when indicated, obtain orders for admit Must be able to maintain strict confidentiality Knowledge and Skills : Knowledge of crisis intervention techniques.
Strong verbal and written communication skills. Significant computer skills, data entry, word processing, and spreadsheet maintenance.
