Position Overview :
The Stroke Program Nurse Navigator plays a vital role in ensuring seamless, multidisciplinary care for stroke patients. Reporting to the Stroke Program Coordinator, this role is responsible for providing expert nursing consultation and practice oversight while acting as a liaison between medical staff, nursing teams, patient families, and support services.
The Nurse Navigator leads the coordination of care based on evidence-based practices and oversees all aspects of patient care delivery, including post-discharge follow-up.
This position also plays a key role in stroke education and works closely with stakeholders to maintain stroke program certification requirements through data review and performance improvement initiatives.
The patient experience is a central focus in all aspects of care.
Key Responsibilities :
- Provide expert nursing consultation and oversight of multidisciplinary stroke care.
- Collaborate with physicians, nurses, families, and support teams to deliver comprehensive stroke patient care.
- Educate patients and staff on stroke care and best practices.
- Ensure compliance with stroke program certification standards, including data review and quality improvement initiatives.
- Facilitate post-discharge follow-up care and patient support.
Qualifications :
- CA Registered Nurse (RN) license is required.
- Nurse Practitioner licensure with a furnishing number in California.
- DEA furnishing certificate is required.
- Certification in BLS and ACLS is mandatory.
- NIHSS certification within 3 months of hire / transfer.
- ASLS certification within 3 months of hire / transfer.
- ENLS certification within 6 months of hire / transfer.
- CNRN or NVRN certification is required.
- Master’s degree in nursing is required.
- SCRN certification is preferred.
We welcome Nurse Practitioners with limited experience, including new graduates, if they have a strong background in ICU, neuro, or emergency department (ED) nursing.
Compensation :
The base salary for this position ranges from $123,843 to $197,163 per year (USD), depending on individual qualifications and experience.
