Position Summary

The Lead LPN is responsible for the proper and adequate day to day functioning of the clinic. This position ensures efficient patient flow, patient care, patient issues and also the supervision of the nursing staff, including staff performance and staff issues.

  • Maintains a professional environment in a multi provider, multidiscipline organization.
  • Maintains a patient centered environment including but not limited to :
  • Provision of care and advocate for patients of all ages and their families based on prudent nursing judgment and acceptable standards of practice.
  • Efficient patient flow, assisting providers and other staff.
  • Perform tasks and procedures within the scope of a Licensed Practical Nurse (L.P.N) and legal guidelines for the Nurse Practice Act.
  • Perform nursing tasks in all clinical areas.

QUALIFICATIONS :

  • Graduate of an accredited school of nursing.
  • Current state licensure to practice in Massachusetts.
  • Cardio-pulmonary resuscitation (C.P.R.) certified.
  • Maintains licensure and certifications.
  • Bilingual preferred.

PRINCIPAL DUTIES AND RESPONSIBILITIES

PROFESSIONAL BEHAVIOR :

  • Attends 75% of nursing staff meetings, supervisor meetings and other health center meetings, and in-house in-services.

SUPERVISORY

RESPONSIBLE FOR :

  • Daily tasks in the nursing department.
  • Direct supervision of R.N.’s, L.P.N.’s, Medical Assistants, Phlebotomists, students and volunteers.
  • Participates in orientation of new staff, students, and volunteers.
  • Participation in nursing staff education and training.
  • Participation in any nursing staff disciplinary action.
  • Maintaining adequate staffing in conjunction with the budget.
  • Assigning and re-assigns tasks to the nursing staff on a daily basis and as necessary.
  • Is responsible for being on duty as the licensed coverage in the event that other licensed staff is not available.
  • Maintaining a clean, neat, and ordeal medication room.
  • Interacts with drug representatives.
  • Oversees flow of patients into clinical area, re-assigning staff and rooms as needed.
  • Completion of the staff’s annual evaluation.
  • Oversees clinical tracking.
  • Ordering medical supplies.
  • Maintaining Medical Equipment.

TASKS : - including but not limited to :

As LEAD :

  • The liaison for the school system, department for children and families (DCF), The Department of Public Health (DPH), The Board of Health.
  • Completion of payroll for the PEDI Nursing Staff.
  • Assuring lunch breaks for the PEDI Nursing Staff.
  • Vaccine manager for the health center
  • Tracks 0-2 year old patients for monitoring of vaccine schedule.
  • Patient preparation for visit
  • Appropriately prepare patient for either a complete physical, sick visit, follow up visit; or a gynecological exam.
  • Vital Signs : blood pressure, temperature, pulse (all point), orthostatic blood pressure and pulse.
  • Obtain interpretation when needed.
  • Chart documentation.
  • Measurements : Height, Weight, and (other measurements as needed).
  • Documentation of reason for visit, medication review, allergy review.
  • Documentation of medication review.
  • Patient tests / treatments
  • Audio acuity testing
  • Ear irrigation
  • Electrocardiogram (EKG)
  • Visual acuity testing
  • Injections- Intramuscular (I.M.); subcutaneous (S.C.), Intradermal (I.D.); Z-track

  • Patient Centered Medical Home (PCMH)
  • Participation in the PCMH includes but is not limited to :
  • Overseeing completion of PCMH work by nursing staff assigned to their floor
  • Delegating follow-up of patient hospital discharges to other nursing staff in the absence of the assigned nurse. Assists with hospital discharges as needed in order to maintain compliance with following-up on hospital discharges within forty-eight (48) business hours.

QUALITY ASSURANCE / IMPROVEMENT :

  • Quality assurance monitoring- participates in the betterment of the health center through studies and reviews as necessary.
  • Goal setting- assess personal, team, and organization goals.
  • Participates in devising, reviewing, and revising policies, procedures and protocols.
  • Assist with ongoing Quality Improvement activities throughout the health center. This includes, but it not limited to conducting Joint Commission tracers, serving on QI teams and committees as needed, assisting with staff education, and performing data reviews.
  • Assist with ensuring staff compliance with HIPAA Privacy and Security Regulations on an ongoing basis through staff monitoring.
  • Attends external conferences and meetings.

WORKING CONDITIONS :

Works in a busy community health center in a downtown business district. Long hours are sometimes required.

PHYSICAL REQUIREMENTS :

Physical demands are requiring lifting of small pieces of equipment, boxes, and / or patients not to exceed forty (40) pounds.

Mostly standing and walking required. Visual acuity is sufficient for frequent reading. Hearing acuity is sufficient for holding conversation with or without audio devices.

This position will receive one-dollar ($1.00) per hour increase above their hourly rate.
