Arizona Home Care and Patient Care Infusion are looking for an independent go-getter with excellent customer service skills to join our team as a Transitional Care Coordinator and represent us in central Phoenix hospitals and facilities.

Our Transitional Care Coordinator will be responsible for the accurate, timely, processing of referrals for Arizona Home Care (homecare) and Patient Care Infusion (home infusion).

  • Minimum 1 year of work experience in a discharge planning role from a facility or hospital or at least 1 year of experience in a Medicare-certified, accredited home health agency, Infusion Pharmacy or related area
  • Current driver’s license
  • Current vehicle insurance

Responsibilities :

1. Successful execution of the referral process (onboarding) including but not limited to : demographics, insurance coverage,

health status, clinical history, physical information, following physician, face to face documentation, etc.

2. Maintains lasting relationships with decision makers, referral sources and physicians.

3. Connects with customers to understand needs and demands of the referral source.

4. Demonstrates strong knowledge and expertise on all service lines.

5. Performs other duties as assigned.

6. Meet or exceeds revenue goals for the Central Territory.
