Job Title : Wound Certified Nurse Practitioner AGNP / CNP / FNP

Position Overview :
Twin Cities Physicians is seeking a dedicated and compassionate Wound Certified Nurse Practitioner to join our team. In this role, you will drive to Senior Living Communities to provide on-site primary care to residents.
This includes facilitating the patient's plan of care, managing chronic conditions, providing advanced treatment, and engaging in end-of-life planning.
Your role will be pivotal in coordinating with the interdisciplinary team, reconciling medications, and ordering labs. Effective communication with physicians, office support staff, community staff, and patients' families is essential to ensure comprehensive care.
Responsibilities :
- Primary Care Provision : Drive to Senior Living Communities to deliver on-site primary care services to residents.
- Care Coordination : Facilitate the patient's plan of care, including chronic care management, advanced treatment, and end-of-life planning.
- Interdisciplinary Collaboration : Coordinate with the interdisciplinary team, reconcile medications, and order labs as needed.
- Communication : Maintain clear and regular communication with physicians, office support staff, community staff, and patients' families.
- Wound Care : Provide specialized wound care and treatment, ensuring the best possible outcomes for residents.
- Documentation : Ensure accurate and timely documentation of patient assessments, care plans, and treatments in electronic medical records (EMR).
Qualifications :
- Licensed as a Certified Nurse Practitioner / CNP in the State of Minnesota.
- Certification in wound care (WCC, CWOCN, CWS, or similar).
- Experience in geriatrics and / or primary care.
- Valid driver’s license and current auto insurance.
- Passion for serving the senior population.
- Strong interpersonal communication skills.
- Proficiency in using electronic medical records and other healthcare technology.
