SUMMARY :
We are looking for a case manager to join our growing team!
JOB LOCATION : Fort Pierce, FL

DUTIES AND RESPONSIBILITIES :
Remains educated and up to date on the program changes and basic operations.
Must always use discretion and good judgement to ensure the good reputation of the organization.
Attends all mandatory meetings assigned by supervisor.
Demonstrates adaptability and flexibility to changes in the work environment.
Builds positive relationships with all other Prevention Central staff members and volunteers.
Screen cases for eligibility and services
Complete juvenile client intake assessments
Complete weekly juvenile client sessions and monthly group sessions.
Make appropriate referrals.
Provide direct client services and supervision.
Conduct case staffing and participate in multidisciplinary staffing.
Participate in peer reviews.
Ability to staff court cases and make court presentations ensuring program complies with Department of Juvenile Justice and the Florida Network requirements.
QUALIFICATIONS :

Ability to learn how to communicate to large and small audiences.
Good verbal and written skills or willingness to learn.
Good interpersonal relation skills or willingness to learn.
Successfully pass a Level II Background Screening.
EDUCATION and / or EXPERIENCE :
A minimum of 5+ years experience with at-risk youth and families and 2 years of case management experience or a Bachelors Degree in Human / Social Service Field.
Knowledgeable of community resources
Ability to compile data, complete reports and maintain appropriate documentation.
Ability to learn or has skills in imputing NETMIS information,
Ability to work with employees at all levels and other agency / organizations.
Positive attitude.
Ability to provide advocacy, linkage, and network services for and on behalf of clients and their families.
Consistent and accurate documentation and appropriate billing
