UH Lakewood Medical Center is located between Kansas City and Lee’s Summit, Missouri, offering residents of Eastern Jackson County access to a modern community hospital with a continuum of outpatient services designed to meet the needs of families.

UH Lakewood specializes in high-risk maternal and child care, and serves as one of the few Kansas City metro area institutions accredited by The Joint Commission in Orthopedics and Palliative Care.

It also is home to a 188-bed, fully accredited long-term care center.

UH Behavioral Health is the KC metro area’s largest, most comprehensive provider of mental health services. The Crossroads Building , located in Kansas City’s vibrant Crossroads Arts District, is its epicenter, providing a broad list of outpatient services.

We also offer psychiatric inpatient care, and we deliver therapeutic services through our community outreach clinics. We are a leader in the Trauma-Informed Care Movement, and provide training, coaching and consulting to outside organizations.

Job Description

Provides supportive services for clients who are experiencing problems as a result of mental illness. Services include triage, risk assessments, ongoing assessment of psychosocial needs, discharge planning, information and referral, client and family education, coordination of services with managed care contacts and / or other service providers.

The community support specialist functions as a member of an interdisciplinary team for client care and works effectively in a multicultural setting.

The community support specialist works under the regular supervision and direction of a QMHP.

Minimum Requirements

Bachelors or Master’s degree in an accredited program in the human service field approved by the State of Missouri. Or possesses a relevant degree and extensive college coursework in a human services field.

Must have good verbal and written communication skill

Good leadership skills Ability to evaluate program outcomes

Ability to work with multicultural staff and clientele.

If driving is a requirement for the employee, he / she must meet the requirements specified in the Fleet Driving Safety and Compliance Policy and must continue to meet these requirements as a condition of employment.
