Job summary

An exciting opportunity has arisen for aCommunity Support Worker to join our Neighbourhood Mental Health Team (NMHT).The NMHTs are at the forefront of providing accessible care, integrating bothprimary and secondary mental health services, in line with NHS long-termobjectives.

There are 5 NeighbourhoodTeams in Herefordshire. The post advertised is to work within the North NMHT. Each NMHT is comprised of Mental HealthPractitioners, Support Workers, Administrators, Clinical Psychologists and aConsultant Psychiatrist.

Additionally, the teams are supported by Link Workersand Vocational Specialists from Mind and Landau, with theaim of providing a holistic, recovery-focused service.

We work closely with ourPrimary Care Networks, demonstrating good relationships and communication withour respective GP partners.

The teams are supportive, with regular MDTmeetings, Individual / group supervisions and team away-days. For moreinformation please contact the Recruiting Manager.

Main duties of the job

To providea range of patients with mental health support in the community, implementingplans of care, utilising personal autonomy and utilising the support of theMulti-disciplinary Team (MDT) when needed, to support individuals with theirongoing recovery.

To workwith a range of agencies within the NMHT and externally, to sign-post patientsto appropriate sources of support or provide short, moderate or longer-terminterventions to support their recovery, based on their clinical need.

The postwill require working with the team to support the running of the service,managing a caseload, attending meetings, and taking on additionalresponsibilities as and when required.

The ability to be flexible and organisedis essential. Experience of working with individuals on graded exposure work ispreferable.

To be able to travel throughout the county asrequired to provide patient care and multi-agency working. To also provide supportto those in a caring role.

Job description

Job responsibilities

To providesupportive treatment of working aged adults with mental health problems,ranging from a primary to a secondary level of need

To workwith internal and external agencies

To travelaround Herefordshire providing support in a variety of clinical settings, aswell as in patients own homes

To managea caseload under the supervision of more senior staff

To attendmeetings required by the service

Tocomplete statutory and mandatory training as required for the role, and keepthis updated as required

To workaccording to Trust values, policies and procedures

To providesupport to carers

Toundertake extra duties as required, depending on the needs of the service

Skills and Abilities

Essential

  • Ability to challenge poor behaviour.

Knowledge

Essential

Knowledge of the different types of mental health problem and presenting symptoms

Desirable

Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation

Qualifications

Essential

  • Level 3 qualification (, A-level, NVQ3) in Care or Level 2 qualification (, GCSE, NVQ2) in care and will be expected to undertake the relevant level 3 qualification on appointment to post
  • Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

Desirable

  • Medication administration qualification
  • Additional qualifications regarding the care of others

Experience

Essential

  • Previous relevant experience working with the client group
  • One to one working
  • Personal care tasks

Desirable

  • Working in collaboration with VCSE service
  • Experience of completing graded exposure interventions and early warning signs work
  • Experience of delivering evidence based psychosocial interventions

Additional Criteria

Essential

  • Will be required to complete breakaway / de-escalation training.
  • Able to meet the travel requirements of the post
