The HOPE intake coordinator is a valued position in our child and family engagement center. This position is often the first contact our children, youth, families, and department of social service workers meet to help them navigate the health care systems.

They have a key role of working with youth and families in foster care, receiving adoption assistance, and former foster youth, their providers, and the department of social service workers who care for these children and youth to help them arrange needed appointments, address social determinant of health needs, navigate customer service concerns, and assess initial health risks.

This position is located within the lower Peninsula of Michigan.

Primary Responsibilities :

  • Responds to Incoming caregiver, adoptive parent, former foster, DSS, and with approval biological parent calls
  • Resolve or refer billing and customer services inquiries for specialty plan members
  • Provide excellent Customer Service to both for specialty plan members and stakeholders
  • Consistently meet established productivity, schedule adherence, and quality standards
  • Receives Care Coordination notification cases for non-clinical assessment / intervention and provides appropriate triage
  • Ensures all potential member needs are identified and forwarded to the appropriate Care Manager or supervisor for risk validation when applicable
  • Provides education related to entrance into care exams and escalated well-child visits per the AAP schedule for foster children
  • Identifies and distributes trauma-informed care and disease management mailings.
  • Processes member and provider mailings as needed
  • Complete member / family assessments as required
  • Handles a caseload of low risk stratified members in foster care, receiving adoption assistance, or former foster youth
  • Performs assessments on low risk members in foster care, receiving adoption assistance, or former foster youth
  • Establishes low risk care plans for members in foster care, receiving adoption assistance, or former foster youth
  • Provides supportive trauma-informed education to help increase family stability in families at risk of disruption
  • Provides transitional life skills education to transitional age youth and young adults
  • Advocates on behalf of the youth and families
  • Other duties as assigned by manager and / or director

Required Qualifications :

  • High School Diploma / GED (or higher)
  • 2+ years of customer service experience or community outreach experience
  • Intermediate level of computer proficiency including MS Word, Excel, and Outlook
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Valid state of Michigan driver’s license with state required insurance and access to reliable transportation
  • Ability to work Monday through Friday between the hours of 8 : 00am and 5 : 00pm
  • Must reside within the lower peninsula of the state of Michigan

Preferred Qualifications :

  • 2+ years working within the child foster care system
  • Experience working within the health care Industry and with health care insurance
  • Experience working in a hospital, physician's office or medical clinic setting
  • A clerical or administrative support background
