Job Details Responsibilities : Provides chemical dependency counseling, intervention and discharge planning as a member of an interdisciplinary team to adult individuals with chemical dependency and / or mental health problems.
Screens potential individuals for admission via telephone / walk-ins to ensure appropriate level of care, using the OASAS LOCADTR tool.
Refers individuals to other referral services if the individual is not appropriate for the programs level of care or if there is no bed availability.
Receives and processes individuals through the admissions process based on unit admission guidelines, including unit orientation, fire safety, dignified contraband search, confidentiality, alcohol, drug, tobacco free policies, and proper adherence to infection control and hygiene standards.
Collects concise, accurate individual Bio-Psychosocial information, completes the Admission Assessment intake form and documents the admitting note.
Develops and documents an individualized Recovery Plan within 24 hours of admission based on presenting problems of individual and with collaboration of individual.
Provides assistance to individuals with appropriate after care linkage, individual crisis counseling, and basic education about the disease process.
Develops educational topics and facilitates group counseling. Plans effective and timely discharges based on utilization review criteria and communicates discharge planning with Program Supervisor / Director.
Observes and monitors individual daily activities, behaviors and moods, and fully documents finding and observations in progress notes.
Monitors and observes individual’s progress toward achieving Recovery Plan goals and documents finding every two (2) days on Recovery Plan Review and daily or as needed in Progress Notes.
Maintains confidentiality of individuals, employees, department and agency information including computer passwords. Prepares and serves snacks (and / or meal services) as assigned.
Provides assistance and education to co-workers and serves as a resource person to non-certified and support staff. Maintains yearly educational requirements set forth by OASAS to allow for timely renewal of credential.
Follows agency policy regarding attendance. Follows agency policy regarding lateness. Complies with dress, appearance standards for position.
Interrupts and reports potential abuse to administrator immediately to administrator. Keeps supervisor apprised of significant issues.
Job requires access to the individuals entire protected health information (PHI), in accordance with the Health Information Privacy and Accountability Act (HIPAA) regulations.
Qualifications : CASAC, LMHC or LMSW with experience in substance abuse required High School diploma required. Bachelor’s degree in Human Service field preferred.
One (1) year working with individuals with chemical dependence and / or psychiatric experience preferred. Maintains yearly educational requirements set forth by OASAS to allow for timely renewal of credential Salary Range USD $24.
69 - USD $38.46 / Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
