Position Summary
Assists in patient’s adjustment to their illness / condition and adherence to the treatment plan, as well as ensuring that they are aware of, and have access to, appropriate community resources.
Includes patients / caregivers in the involvement or formulation and implementation of the patient's plan of care, and interdisciplinary collaboration will be employed to ensure the greatest continuity of care.
Primary Position Responsibilities

- Education and Counseling.
- Maintain resource materials.
- Documentation.
- Collaboration / Conference.
- Other duties as assigned.
Qualifications
- Minimum Education : Required : Master’s Degree in Social Work Preferred : None
- Minimum Years of Experience (Amount, Type and Variation) : Required : Three years social work experience, preferably with older adult population;
experience with community agencies helpful. Preferred : Three years social work experience, preferably in Home Health.
License, Registry or Certification : Required : New Jersey Licensure for Social Work, LSW or LCSW. Valid NJ driver’s license. Preferred : None
Knowledge, Skills and / or Abilities : Required : Strong clinical assessment skills and the knowledge and ability to respond to routine social work related inquiries.
Knowledge of local community resources. Preferred : None
