SCAN Navigator Job Description Job Type : Full Time Reports to : Youth Program Manager Location : Chicago, IL Organization Background : Established in 2002, Precious Blood Ministry of Reconciliation (PBMR) is a faith-based, restorative justice organization in the Back of the Yards / Englewood community.

PBMR is rooted in the spirituality of the Precious Blood, we restore human dignity through hospitality, hope, and healing.

Through a restorative justice philosophy, we seek to build a stronger community. We work as agents of reconciliation to : Build relationships among youth and families impacted by violence and / or conflict Create safe spaces where people can experience radical hospitality, hope, and healing Promote a restorative approach to conflict and build a sense of community.

Position Summary : The Navigator will be responsible for providing accompaniment to at-risk participants that are part of the City of Chicago Service Coordination and Navigation Program (SCAN).

The youth that are included in this program may already have contact with the court systems (criminal, juvenile, and / or foster care) or be known to local community leaders.

• Essential Functions : The essential functions of the position include, but are not limited to, the following : Recruit youth through engagement / referral efforts with the following organizations : Chicago Department of Public Health;
• Cook County Juvenile Probation; Foster Care; Homeless shelters; Chicago Department of Family & Support Services; Chicago Police Department;

and other street outreach organizations Engage in off-site meetings, workshops, and events with partners Create and foster relationships with partners Advocate for Back of the Yards and Englewood neighborhoods as well as for youth and families, when in policy settings Referrals and Connections Communicate with youth and build trust to engage them in services Connection youth to any needed service.

Types include : Mental health services / assessment Education Employment services Health and wellness Housing Ancillary support : transportation;

cell phones / access to a phone line; email addresses / access to WIFI; financial literacy and income, etc. Accompany youth while engaging with services, participate in warm handoffs to ensure the youth is comfortable and in a healthy setting Complete basic intake to help get to know the individual and identify their unique challenges and goals Exhibit clear and supportive goals, plans, and expectations with the participants Assessments will be used to develop case plans and assess areas of strength and priorities based on the youth’s direction and resources (i.

e. substance use treatment, CBT and other mental health services). Report on youth’s progress, challenges, potential collaboration gaps and successes, as well as other ancillary issues which can assist and inform DFSS and programming for youth at the highest risk of violence involvement.

Qualifications : College degree in related field (Preference) Ability to live out of a restorative justice philosophy and use the circle process when needed Skilled in building healthy relationships, maintaining boundaries and valuing confidentiality Willingness to be a good steward of resources (financial, etc.

Ability to effectively communicate through written, verbal, and electronic methods (including various social media applications, text, email, and phone calls) Ability to be a self-starter, problem-solver and dreamer of possibilities Conduct oneself in a professional, warm and approachable manner Organized and able to meet deadlines and multitask Proficient in Microsoft Office Suite and comfortable using computer programs to track participant engagement Able to work in a fast-paced environment High level of interpersonal and conflict resolution skills Valid driver’s license and safe driving record;

preferred personal vehicle with current insurance. Strong leadership Passing a background check is required prior to being hired Trauma-informed and diversity training (Preference) Experience in mentoring at-risk / court involved youth (Preference) Schedule / Travel : At times, this position requires working evenings, weekends, and occasional travel.
