We're currently seeking a motivated and skilled Weekend Admissions Coordinator to join our team and contribute to our mission of delivering high-quality healthcare services.

Job Overview : We are seeking a detail-oriented Admissions Coordinator to join our team. The ideal candidate will have experience in admission roles, particularly in a long-term care center.

As an Admissions Coordinator, you will be responsible for managing patient admissions, reviewing and processing referrals, and providing excellent customer service for Southview Acres and Highland Chateau.

Responsibilities include but are not limited to :

Establish regular and ongoing communication with acute care and home care staff in the community to assist with referrals to Southview Acres and Highland Chateau.

Coordinate patient admissions and discharge processes

Maintain and update patient records using Epic and Point Click Care

Review the referral form from referral source regarding primary diagnosis, medications, treatments and functional capacity of resident.

Assess whether Skilled Nursing Facility has the resources to provide the care required

Verify insurance information

Communicate with the Interdisciplinary Team concerning special needs of residents (equipment, dietary concerns, behavioral, emotional, etc.)

Ensure compliance with HIPAA regulations

Collaborate with medical staff to facilitate urgent care admissions

Other duties as required

Qualifications :

Current and active license by the Minnesota Board of Nursing to practice as a Registered Nurse or as a Licensed Practical NurseFami

• Familiarity with Epic and Point Clic Care is a plus but not required
• Ability to communicate effectively in both verbal and written skills
• Knowledge of insurance sources
• Strong attention to detail and organizational skills
• Ability to maintain confidentiality and adhere to HIPPA guidelines

Job Type : Part Time Weekends

Open to hiring for every weekend or every other weekend*

Pay : $27-$36.50
