The Patient Care Coordinator plays a vital role in ensuring the smooth functioning of our clinical operations. Responsibilities include managing patient inquiries, maintaining relationships with families, scheduling appointments, documenting patient interactions, and assisting with office maintenance.
Additionally, the Patient Care Coordinator will serve as the first point of contact for new patients, triage patient calls to ensure same-day responses, manage the schedule of multiple clinicians, and ensure adherence to care plans.
Job Responsibilities :
- Manage inquiries promptly, providing comprehensive information and polite responses.
- Utilize patient portal for direct communication, ensuring timely responses.
- Develop strong relationships with care plan families, addressing their needs professionally.
- Ensure delivery of promised services outlined in care plans.
- Schedule patient sessions with 100% compliance to care plan recommendations.
- Coordinate communications with outside providers and families regarding care plans.
- Maintain accurate documentation and patient communication logs.
- Assist with office cleanliness and maintenance, reporting any issues promptly.
- Manage inquiries from new patients, enter referral information, and schedule intakes efficiently.
Qualifications :
- Preferred experience in scheduling and clinical administrative assistance.
- Experience working with a CRM system is desirable.
- Proficiency in speaking with patients over the phone while maintaining confidentiality.
- Experience in healthcare administration preferred.
