YOUR IMPACT AND RESPONSIBILITIES : Counsels families and patients considering Hospice services and helps with signing legal documentation to initiate Hospice services.
Assesses emotional factors related to terminal illness. Helps physicians and other interdisciplinary group members in recognizing and understanding the social / mental stress and / or disorder that exacerbates the symptoms related to terminal illness.
Assesses the patient and family / caregiver psychosocial status, potential for risk of suicide, and / or abuse or neglect.
Assesses environmental resources and obstacles to maintaining safety. Participates in the development and revision of the plan of care.
Provides social services, including short-term individual counseling, crisis intervention, and assistance in providing information and preparation of advance directives.
Helps with funeral planning issues and transfer of responsibilities regarding fiscal, legal, and health care decisions. Involves the patient and family / caregiver in the plan of care.
Evaluates patient and family / caregiver response to psychosocial interventions. Assesses caregiver’s ability to function adequately.
Assesses need for counseling related to risk assessment for pathological grief. Assesses special needs related to cultural diversity including communication, space, role of family members, and special traditions.
Identifies the developmental level of patient and family / caregiver and obstacles to learning or ability to participate in the care of the patient.
Addresses patient and family / caregiver questions. Identifies obstacles to compliance and helps in understanding goals of interventions.
Identifies support systems available to reduce stress and facilitate coping with end-of-life care. Evaluates for long-term care when appropriate and assesses ability to accept change in level of care.
Identifies patient and family / caregiver needs when discharged or when level of care changes. Assists Home Health patients / families with community resources, planning, counseling, and other tasks Performs other duties as assigned.
QUALIFICATIONS FOR SUCCESS : Master's degree in social work. Current CPR / First Aid certification through the American Heart Association.
Excellent verbal and written skills. Must have a genuine desire to work with the elderly and / or disabled. Communication and interpersonal skills to develop relationships with clients, staff, and providers. Powered by JazzHR
