Job Purpose

The purpose of this role is to provide effective leadership and supervision of day center staff, ensuring the smooth and efficient operation of daily activities.

This position involves active participation in Interdisciplinary Team (IDT) meetings, serving as a leader to ensure that all practices align with DHCS (Department of Health Care Services) and CMS (Centers for Medicare & Medicaid Services) guidelines.

Responsibilities include collaborating with team members to develop and implement policies and procedures for the PACE (Program of All-Inclusive Care for the Elderly) program, overseeing daily operations to maintain compliance with regulatory standards, and preparing for audits to ensure readiness and adherence to all relevant guidelines.

Duties and Responsibilities

  • Directs, supervises and evaluates the performance of the Personal Care Assistant, and maintains a matrix reporting / supervisory relationship with staff involved in participant care / experience at the Day Center.
  • Interviews, recommends for hire day center staff.
  • Prepares periodic reports and assists with QI projects to review staff performance as related to planning, quality of work, procedures, and participant care.
  • In collaboration with other site managers and supervisors, established and maintains uniform, consistent delivery of care and services for participants, including internal and external appointments with transportation needs.
  • Arranges personal care Assistant (PCA) coverage for all areas of responsibility.
  • Works with supervisory staff and site manager to assure that cultural needs of participants are met in the day center.
  • Works closely with Operations Director and Social Services to ensure recreational programming is therapeutic, outcome based, based on best practice data, and culturally competent.
  • Participates in interdisciplinary team meetings and reports regularly on personal care services provided and the self-care abilities of participants, including changes in participants’ condition.

Works with the interdisciplinary team to recommend and initiate changes in participant plans of care and implements and communicates changes to Personal Care staff.

  • Works in conjunction with Home Care Coordinator to arrange weekend and home care assignments of personal care staff.
  • Coordinates and facilitates the welcoming and orientation of participants during assessments and after enrollment.
  • Regularly meets with and recognizes strengths and accomplishments of personal care staff; provides direction and supports to personal care staff daily.
  • Holds formal monthly and as needed meetings with PCAs and records minutes.
  • Monitors transportation services and resolves problems as necessary with Transportation Coordinator.
  • Monitor and manage the Dietitian and monitor matters related to food / nutritional services within the Adult Day Health Center.
  • In consultation with other management staff, develops and implements policies, procedures and standards for the Adult Day Health Center.

Communicates changes to staff, monitors outcomes and makes changes as needed.

  • Assumes a leadership role in the development, implementation and refinement of day center systems and coordination of schedules which involves rigorous communication and collaboration with all departments, including transportation, the clinic, and rehabilitation therapy, recreation therapy, dining services, home care, social work, maintenance and environmental services.
  • Interfaces with program participants, families, contracted providers and the public in regard to issues related to the routine operation of the Day Center.
  • Monitors the physical environment of the Adult Day Health Center in conjunction with assigned staff, to ensure a safe, orderly, and sanitary environment is consistently maintained for participants and staff.

Notifies appropriate maintenance or housekeeping staff of problems or concerns as indicated; consults with Operations Director regarding significant repair / replacement needs.

  • Reviews safety issues, including participant falls and incidents as they occur, and reports to appropriate personnel, and appropriate committees.
  • Assists the Operations Director with budgeting, monitoring and analyzing site performance trends in utilization, financial and participant outcomes.
  • Collaborates with Operations Director and clinic manager to investigate and resolve grievances, service delivery concerns, and incidents relating to Adult Day Health Center operations.
  • Other duties as assigned.

Qualifications

Education :

  • Bachelor’s degree in a health care or human service related field.
  • Current BLS / First Aide preferred.

Experience :

  • Three years of experience in human service delivery with the frail, elderly, individuals with physical and / or developmental disabilities.
  • Two years of supervisory experience.

Skills and Knowledge :

  • Knowledge of PACE program.
  • Excellent organizational, interpersonal and presentation skills.
  • Excellent verbal and written communication skills.
  • Proficient in Microsoft Office software applications.
  • Ability to lead and motivate individuals and groups of people, including Associates, management, and physicians.
  • Ability to work without close supervision or professional guidance and to exercise independent judgment.
  • Knowledge of physical, mental and social needs of frail older adults.
  • Effective skills in physical assessment and chronic disease management for frail older adults.
  • Effective listening and oral and written communication skills.
  • Demonstrated ability to work effectively within the interdisciplinary team setting.
  • Computer proficiency.
  • Able to manage changing priorities per participant needs.
  • Strong organizational skills.
  • Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.

Other :

  • Must be able to work required schedule.
  • Requires physical strength to perform essential functions of the job.
  • Occasional travel between sites, nursing / group homes and to members’ homes required.
  • Requires use of personal vehicle.
  • Requires valid driver’s license.
  • Requires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage : $100,000 / $300,000 personal liability and $100,000 property damage.
  • May require use of personal cell phone for business purposes (may be eligible for stipend)

Physical Demands

  • Must be able to remain in a stationary position 50% of the time.
  • Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
  • Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.
  • The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.
