Key Responsibilities : Assist with care management for high-risk members, including comprehensive needs assessmentsDevelop and implement individualized, person-centered care plans in collaboration with internal teams and community resourcesProvide referrals and linkages to appropriate services based on member needs and acceptanceRequired Qualifications : Bachelor's degree in social work, psychology, counseling, gerontology nursing, or health & human services, or equivalent experienceAt least two years of direct service experience with older adults or persons with disabilitiesExperience with MS Office applications, including Word, Excel, and OutlookAccess to a designated quiet workspace with secure internet for telecommutingAbility to secure Protected Health Information (PHI) in a home workspace
