An array of services supports families from cradle to college to alleviate societal challenges that can interrupt a child, adolescent or young adults' abilities to thrive and maximize their potential.

Children's Aid offers a continuum of short-term evidence-based interventions of varying levels of intensity, in four boroughs.

The preventive services continuum consists of Family Support, therapeutic and Family Assessment Program (FAP) program services as well as the Safe Way Forward Program for families impacted by Domestic Violence.

Our preventive service interventions are charged with the responsibility to avert current, future or repeat involvement with child welfare, juvenile justice systems.

Program Description : The Children's Aid A Safe Way Forward Program (SWF) provides child welfare involved families affected by intimate partner violence (IPV) with comprehensive general prevention services, including case management, and therapeutic intervention services to address the IPV.

This joint prevention-clinical service model offers comprehensive trauma-informed services to the whole family, including the survivor of IPV, the person causing harm (PCH), and children exposed to the IPV / DV in the home.

Our range of services includes in-home visitation, crisis intervention, case management, advocacy, trauma-informed individual counseling, group counseling / education, and referral to other appropriate services.

The SWF program serves 65 families residing in the Bronx.

Position Summary : The Family Team Conference (FTC) Facilitator will report to the Safe Way Forward (SWF) Program Director and work closely with all program staff to schedule and facilitate Family Team Conferences;

coordinate meetings with providers and families to review risks and protective factors as well as service goals and next steps.

FTCs will take place initially and throughout the life of the case. The FTC Facilitator will also maintain reports on occurrence of FTCs for the purposes of Quality Assurance (QA) compliance.

Families are referred to SWF by the Administration for Children's Services (ACS).

Essential Duties :

  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Facilitate Family Team Conferences (FTCs) (Survivor and Person Causing Harm) for families in the SWF program.
  • Oversee the scheduling and coordinating of FTCs / Service Termination Conferences (STCs) and other conferences, including planning sessions.
  • Liaison with various ACS units.
  • Prepare statistical reports with regards FTC standards and policies.
  • Participate in Administrative team meetings, staff training, and planning sessions.
  • Liaison with the CQI manager and designated leadership to ensure the program meets all conferencing mandates.
  • Using excel, track, record and coordinate all conferences; Document FTC outcomes in Connections and PROMIS.
  • Participate in and conduct ongoing trainings for staff on FTCs.
  • Provide shared coverage for the SWF hotline.
  • Follow all FTC protocols and related tasks.
  • Provide Therapeutic Case Planners (TCP) staff with timely documentation of conferencing Agreements and progress notes.
  • Work flexible hours on weekdays and availability evenings and on Saturdays, as needed.
  • Perform other duties as assigned.

Minimum Qualifications :

Master's degree in social work, mental health, or other related field required; licensure preferred

Key Competencies :

  • Minimum of 2 years post MA / MSW experience preferred
  • Fluent Bilingual English / Spanish preferred
  • Experience with families in crisis and at risk of child placement due to neglect / abuse;
  • Experience working with families impacted by IPV
  • Thorough working knowledge of ACS Prevention System and Standards preferred
  • Knowledge of ACS Connections and PROMIS systems;
  • Excellent oral and written communication skills
  • Working knowledge of Excel
  • Excellent organizational and management skills;
  • Excellent interpersonal and group work skills;
  • Ability to work under pressure;
  • Ability to connect and have empathy for survivor and persons causing harms;
  • Experience facilitating training preferred but not required
  • Trained in the FTC model preferred
