Pharmacy Operations is responsible for the safe, efficient and effective coordination of Cardinal Health's pharmacy operations that service acute care hospitals, hospital retail customers, ambulatory care and alternate site facilities, oncology and cardiology practices as well as retail customers.
Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes.
Under general direction of the System Clinical Pharmacy Director, the Clinical Operations Pharmacist will provide comprehensive pharmaceutical care as defined by the State Office of Pharmacy Services to patients admitted to Western Massachusetts Hospital.
This Clinical Operations Pharmacist will report to Western Massachusetts Hospital located in Westfield, MA, twice weekly on Tuesdays and Thursdays, 8 : 00am-4 : 30pm.
Participate in quarterly interdisciplinary patient and team meetings to provide patient medication updates and assist in the creation and modification of pharmacotherapy treatment and monitoring plans.
Conduct drug regimen reviews for patients quarterly and as needed to ensure continual cost-effective, clinically appropriate, evidence-based treatment.
Serve as a reliable drug information resource to interpret medication therapy for clinical appropriateness and safety including, but not limited to, treatment indication, dosing, duration of therapy, drug-drug / drug-disease / drug-food interactions, adverse effects, therapeutic duplication, therapeutic alternatives, medication compliance, etc.
Perform operational duties to ensure medication access across the care continuum.
Assist with abstract reviews for new admission medication screening.
Document patient-specific clinical interventions and associated cost-savings.
Assist in hospital formulary and inventory management.
Send recurring deliverable documents and updates on activities to the System Clinical Pharmacy Director.
Send recurring deliverable reports and documents to the system Quality Assurance team.
Provide recurring reports to local Quality Assurance team and providers.
Provide in-services and educate healthcare professionals and patients on pharmacotherapy-related topics to promote evidence-based, patient-centered, cost-effective, and safe utilization of medications.
Provide pharmacy orientation for new nursing and respiratory therapist staff.
Participate actively in interdisciplinary committees to improve patient care, medication safety, and regulatory compliance.
Participate in scholarly activities, site-specific performance improvement projects, medication use evaluations, and financial initiatives to contribute to the advancement of pharmacy practice.
Precept Pharmacy Students from local colleges of pharmacy.
Maintain safety, integrity, and professionalism at all times, to include appropriate communication with coworkers.
Organizes and prioritizes work assignments.
Answers the telephone, identifying self and department; directs calls to appropriate personnel.
Keeps pharmacy areas clean, neat, and well organized; Keeps pharmacy equipment clean.
Willing to modify work schedule to accommodate staffing needs of facility.
Perform other duties as assigned.
Bachelors degree in pharmacy or Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school of pharmacy.
Must be a good standing licensed Pharmacist in the state of Massachusetts.
Completion of an ASHP-accredited PGY1 Pharmacy Residency, or a minimum of 3 years of experience in a clinical acute care setting highly preferred.
Board certification in pharmacotherapy preferred.
Experience with the medication formulary process and assessment of appropriate medication use (i.e., P&T, MUEs, etc.).
Experience with drug formulary management is preferred.
Knowledge of federal and state laws and regulations regarding pharmacy practice and protected health information.
Understanding of institutional policies / procedures including standard precautions and infection control and prevention.
Effective interprofessional and interprofessional communication skills and ability to work in collaborative environments.
Time management skills and ability to effectively handle concomitant responsibilities.
Customer / vendor credentialing is required (this may include vaccinations). More details will be provided if you are selected for an interview.
